0125240560 2019-04-04 Production Wells No Violations My inspection found the operator did not correct the compliance issues cited in the Compliance Notice issued on 05-24-16. The compliance issues are as follows: The well is not legibly identified andThe separator has not been moved to the minimum distance of 50 ft. from the wellhead. The contamination at the brine drain was not located. The well is equipped with a pumpjack and all the valves are in the closed position. The tubing head is leaking gas. The fence is no longer locked at the well entry. The tank battery consists of a steel production tank, a steel brine tank, a vertical separator and a meter without a chart.

1756841371 2016-05-17 UrbanProduction Wells Violations Noted The Arthur # 1 well was completed in 10 1983. The well was constructed with 402 of 8.625 surface casing and 3767 of 4.5 production casing. A review of the RBDMS data base indicated that the Operator has not reported production volumes for the calendar year of 2015. Access was gained through a locked gate off of Shade Road. The tank battery consisted of a 100 barrel oil tank, 50 barrel brine tank, vertical separator and a gas measurement meter ( no chart ). The separator was 30 from the wellhead and shall be moved to a minimum of 50 per 1501:9-9-05 OAC. The tank battery was fenced, locked and surrounded by an earthen dike. The load lines were secured with flat plugs. There was ground contamination at the brine drain. The required I.D. signage did not have the current emergency contact number. Two ( 2) joints of pipe were laying outside the tank battery. The fencing for the tank battery continues around to encompass the wellhead. The well is operated as an electric pump jack. The well was overgrown with vegetation and restricting access. The well appeared to have parted rods as there was 50 of polished rod exposed on a unit that had a 42 stroke length. The pumpingunit had not been operable for an extended period of time as evidenced by the trees growing up through the belts. In order to bring the Arthur #1 well into compliance the Owner shall : 1.) Post the I.D. signage with the current contact information, 2.) Report production volumes for the year of 2015, 3.) remove and replace contaminated soil and gravel at the brine drain, 4.) remove all piping and equipment not instrumental to production activities, 5.) Owner shall move separator to a distance from the wellhead that is compliant with 1501:9-9-05. The Owner shall contact the DOGRM Inspector Bob Roush within five (5) days after receiving this Notice of Compliance.
