0000626953 2005-02-09 Production Wells Violations Noted There has been no follow-up since the leak was detected, exposed and clamped. Free oil was found in the adjoining drainage ditch. I called Tim McConahay with Belden and told him to send a water truck, sorbent material and a mini-hoe to address the problem.
0691896925 2005-01-21 Production Wells Violations Noted I received a call from Jim Erwin with OEPA-DERR reporting a pipline leak at 2350 Boston Mills Road. He received a call from the Richfield Fire Department. The Belden & Blake pumper was on site and reported that the line leak was the salesline from the Lowe and Novicky wells, which he shut in. Approximately 10 gallons of crude oil was released with the natural gas. The affected area was approximately 10 by 8. I spoke with Tim McConahay (Belden & Blake Supervisor) who said that a water truck and mini-hoe were enroute to recover any released liquids and repair the line leak. On this day, the leak was repaired and the soil was mounded over the excavation. The snow and ice cover prevented a thorough examination of the adjoining road ditch. The site will be revisited as the snow melts.

1052586590 2015-06-04 Production Wells No Violations I inspected the Lowe Unit # 1 well and tank battery as a follow up to the oil spill in the tank battery on 3/31/2015. My inspection reveal that the entire tank had dissembled and removed. The former tank battery was free of any contamination and native vegetation was growing. The pump jack had also been removed. I spoke with Jeff Moore ( Moore Resources ) and he informed me that a plugging permit had been applied for and the Lowe Unit # 1 is scheduled to be plugged in August 2015. No violations.
1109258465 2005-02-14 Production Wells Violations Noted The issues raised in the February 9 inspection have been addressed. The violation will be corrected as soon as the weather permits reclaiming the excavated area where the line leak was exposed and clamped.
1790860386 2005-04-19 Production Wells Violations Noted It appears likely that 1) more oil was released than was reported and 2) when Belden exposed and clamped the leaking salesline, they placed the impacted soil back in the excavation. Oil is still leaching to the surface and affecting the adjoining drainage ditch. I contacted Tim McConahay and told him that if oil continued to leach, the impacted soil would have to be removed and disposed.
1833480136 2015-03-31 UrbanProduction Wells No Violations I inspected this well on 3.31.2015. On 3.30.2015 @ 1949 hrs. I received a phone call from Jeff Moore ( Moore Resources ) and Steve Terpeck ( landowner ) concerning spill that occurred five ( 5) days previously. According to Mr. Moore the well was produced by a pumping unit for several cycles per day and shut off utilizing a electric timer. On the day ( 3.27.2015 ) of discovery of the spill the timer failed and the pumping continued to operate, filling the 140 barrel storage tank and overflowing into the diked area. I have requested opening and closing gauges ( well tenders ) of the tank to determine fluid volumes. The tank battery is surrounded by an earthen dike which did not appear to be breached. I looked for a dike drain, but could not locate one. The spilled fluid drained through the porous tank battery base until it made contact with a layer of yellow - grey clay. Upon my arrival on location I saw oil absorbent diapers and Peat Sorb spread on the west side of the tank battery, both inside the dike and out. Moore Resources was on location with a mini track hoe and water truck. The water truck removed the surface fluids on the west side of the tank battery and draining the storage tank. I tested the surface fluids in four (4) areas west of the tank battery using Quantabs ( low range, exp. 5/2015 ) and the results were 1.) 41 ppm ( Cl ) @ 41.26286, -81.61662 , 2.) 78 ppm ( Cl ) @ 41.26373, -81.61701, 3.) 96 ppm ( Cl ) @ 41.26378, -81.61694, 4.) 138 ppm @ 41.26376, -81.61700. The pH of the samples ranged 6 to 7. The samples were taken farthest to closet to the 140 barrel tank. A ditch was cut along the west side of the dike to mitigate any further migration of fluids. I noticed in the freshly cut ditch that crude oil was seeping into the ditch. The seepage was at the topsoil - clay layer interface. Moore Resources dug three (3) gathering holes to determine the extent of migration. The oil was found in only one (1) hole and was located 15 NW of the tank. A sump pu

1915843722 2016-06-15 UrbanProduction Wells No Violations After setting the surface plug on 6/14/2016 I inspected the Lowe Unit #1 to confirm that the well was static and the height of the cement. My inspection found that the cement top was 55 down from the Braden head and the well was static. No violations.
