1419361911 2014-12-18 UrbanProduction Wells No Violations I was on location to conduct a status check of this well. The well is of plunger lift construction on it. The valves were open on the well and was in production. The tank battery consists of a 50 BBL production tank, mechanical separator and a 1500 gallon poly brine tank. There were two separate pick-up lines that came off of the storage tank and brine tank. The pick-up lines had bull plugs in them. The chart box on the mechanical separator had been changed on 12-18-14. The tank battery was diked but there was a hole through the dike on the east corner. Standing water was observed within the diked area. The tank battery was fenced but the man door was not locked. The fencing around the wellhead was in place but had been cut to create an opening large enough to walk through with no door to cover the opening. The well is properly identified with a clear, legibly marked well identification sign on the storage tank. The lease road is stable and drivable. The operator will be contacted in regards to the following issues :

1) Locks installed on the man door of the tank battery 2) Repair the fencing around the wellhead 3) Drain the water from diked area and repair the hole in the dike

1487789480 2017-02-22 Administrative Inspection No Violations I inspected the pipeline system for the plugged Hazenfield-Bourgois # 1 due to a gas leak complaint ( # 5538 ) filed by the Hudson Fire Department. The gas sales line crossed Stow Road at the GPS coordinates at 41.20588, -81.41118. A gas leak was noticed by an excavation crew working on the property at 5457 Stow Rd., Hudson, Ohio 44236. I called Tim McConaha ( Enervest Operating ) whom was transporting the natural gas prior to plugging. A pipeline crew was deployed to locate the sales line and isolate the back feeding natural gas. I requested that another well ( Turnblacer-Yeskoo # 1, 34-153-2-1322 ), on the same line, be shut in until the back feeding path could be isolated. OUPS locating service was notified and will mark existing utilities in the area. With the Turnblacer-Yeskoo # 1 shut in, I did not detect any natural gas leaking where previous noted. No violations identified during this inspection.
1488373301 2017-03-21 Plug / Plug Back No Violations

-1322 when the well cycles. Mr. Crookham stated he would shut-in the well and that Enervest would begin locating the sales line tomorrow, February 22, 2017. I contacted ODNR Inspector Bob Roush and updated him concerning complaint #5538. Mr. Roush stated he would follow-up with Enervest concerning the complaint.

1538091936 2015-10-27 UrbanProduction Wells No Violations After the plugging of the Hazenfield-Bourgois # 1, I inspected the cement top height and condition. I found the well static and the height of the cement top to be at eight (8) feet down from grade. I requested that additional Sakrete be used to bring the cement top up within five ( 5 ) feet of the top of the 8.625 casing head prior to affixing the required identifing plate. No violations were identified during this inspection.
1582887169 2017-05-17 Non Well No Violations I was on location to assess complaint #5538. I found no gas leak. I spoke with Kyle Ahbel, field supervisor for Enervest. Mr. Ahbel stated the flow line that was backfeeding and causing the gas leak was dug up and capped. I called and left a message with Mr. Alexander concerning my findings and for a return call.
1761166642 2015-10-26 UrbanProduction Wells No Violations I was on the Hazenfield-Bourgeois # 1 location to witness the plugging and abandonment of the well. Petroset Cementing Services (PCS) was on location to set the plug for the Clinton sand interval. The perforated interval was from 3962 to 4060. Berry Well Service (BWS) was the plugging contractor. Berry swabbed the well from ~ 3200 down to 3800 to reduce displacement pressure. PCS mixed a cement slurry consisting of 30 sacks of Class A blended with 2% CaCl and displaced it with 59 barrels of water @ 900 psi. I scaled the cement at 14.6 ppg ( Class A blended with 2% CaCl ). Calculated top of cement was 3750. Well was shut in over the weekend. No violations were identified during this inspection.
