0748204764 2014-10-23 UrbanProduction Wells Violations Noted This inspection is a follow up to Inspection # 2061133620. In the previous report I noted that the required signage needed replaced and the access gate needed a chain to secure the tank battery. The hasp on the gate will not allow to securely fasten the lock. The well and tank battery is located adjacent to Kreighbaum Ballfields. I spoke with Mr. Mike Cutter concerning the deficiencies who said it would be remedied and ask for some time for the signage, because the adhesive would not stick. We agreed on a 30 day period to replace the I D signage. Upon my current inspection I noted that none of the previous deficiencies had been corrected and a new violation had occurred. The well is operated as a plunger lift well with 1.5 tubing and 4.5 casing. The well is fenced and secured. The motor valve on the wellhead is leaking and needs repaired. The tank battery consists of a 210 barrel oil tank , 25 barrel poly brine tank and 2408 mechanical separator with orifice gas meter. The 210 barrel tank is leaking oil either out of the heating tube or the bottom weld seam has rotted. The oil has contaminated the ground between the brine tank and the oil tank. The signage needs replaced and a chain utilized to secure the tank battery. In order to correct the multiple violations Cutter Exploration shall : ( 1 ) Post the required Signage, ( 2 ) Remove the leaking tank , remove all contaminated soils within the tank battery and replace oil tank with a suitable, leak - free tank, ( 3 ) Placed a chain on the access gate to secure entrance from unauthorized personell. All violations shall be corrected and approved by the DOGRM Inspector by 12/1/2014.

1235719370 2015-03-11 UrbanProduction Wells No Violations I inspected the Standard Slag # 5 on 3/11/2015. The leaking 210 barrel storage has been corrected by sealing off the heating tubes with flat adjustable plugs. The I.D. signage has not been replaced and the security issue with the access gate on the tank battery has not been remedied. The gate was wide open ( with a lock in the hasp ). The tank battery is located in Kreighbaum Ballfields. A chain is required to secure the gate. Until the I.D. is in place and the gate to the tank battery is secure the Compliance Notice ( # 748204764 ) shall remain.
2061133620 2014-02-26 UrbanProduction Wells No Violations I inspected this well and tank battery on 2/26/2014. Upon my entry I noticed that the required I.D. sign was illegible and needed replaced. The locking hasp did not provide adequate security as one could open the gate while locked. The tank battery consisted of a 210 Bbl. Stock tank, 25 Bbl. Brine tank and 2408 Horizontal separator with Gas orfice meter. The well is operated as a plunger lift well. It is constructed of 4.5 production casing with 1.5 tubing inside. All equipment and fittings were leak free. The Demco valve on the 1.5 appeared to be closed and the chart on the meter was not reporting production ( RBDMS reported production continuous production since 1988). I spoke with Mike Cutter about the deficiencies I found. I asked to use a chain around the gate for security reasons ( well is located next to public ball fields, but on private ground ), install the required I.D. with the proper information listed ( weather allowing ). Mr. Cutter advised that the well is shut in due to all tanks are full and awaiting brine hauled off and should be back in production within 30 days. I requested that the tank I.D. signage shall be replaced as soon as the weather warms, to allow the adhesive to adhere and the chain securing the gate should be on within 30 days ( 03/ 36/2014 ).
