0406966670 | 2014-11-10 | Production Wells | No Violations | I inspected this producting property on 11/10/2014. The well has had continuous commerical production ( 1509.062 ) since its inception. The well is operated as a Plunger Lift well with 1.5 tubing inside of 4.5 casing. The well head is constructed in the traditional Plunger Lift configuration. The well head was leak free. The Tank battery consists of a 210 barrel Oil Tank, 100 barrel brine tank. Both tanks were in good condition and liquid tight. A mechanical horizontal separator ( without a gas measurement meter ) was employed for phase separation. The fittings and piping of the tank battery was found to be absent of leaks. Both the well head and tank battery were fenced and secure. Bull plugs were employed in the load lines. No violations were identified during this inspection. |
1485875836 | 2017-01-30 | Plug / Plug Back | No Violations | Bishop Well Service (BWS) was the plugging contracor and Petroset Cementing Services (PCS) supplied the cementing and pumping services. The 4.5 casing had a gauge which read 400 psi. PCS connected to the 1.5 tubing and pumped 10 barrels ( capacity ) of the tubing and pumped 50 barrels down the 4.5 casing to kill the flow from the well. Three (3) additional joints of 1.5 tubing were added to the tubing string to insure the Clinton plug was below the formation. PCS mixed and displaced a cement slurry consisting of 30 sacks of Class A cement with 2% bBentonite gel blended. The cement slurry was displaced through the interval 3826 to 3400 through the 1.5 tubing and the well shut in for two (2) hours. After removing 113 joints of the 1.5 tubing BWS lowered the ripping tool in an effort to assess the top of cement depth. The rig found the fluid level in the casing at 1600 and went to 3745 without any signs of a cement plug. PCS reconnected to the 4.5 casing to evaluate the ability to pump and displace a second Clinton plug by a Bullhead method. PCS pumped approximately 50 barrels into formation at 1000 psi, indicating that some perfs were open. Decision was approved, by me, to squeeze 30 more sack of cement slurry into the open perfs and shut in the well overnight. PCS mixed and displaced a 30 sack plug to 3400 at pressures approaching 2000 psi. After displaement was complete the well was shut in. No violations identified. |

Petroset pumped 80 barrels of fresh water down 1.5 tubing and 4.5 casing to kill well (500 psi) . Bishop Well Service ran 3 joints of 1.5 31 long tubing a total of 93 a total of 3826 to get pipe below bottom perfs. ( Clinton Formation ). Petroset pumped 30 sack of class A cement with 2% gel. Plug was displaced with 8 bbls of water from 3826 to 3400. Bishop Well Service pulled the 1.5 tubing out and rigged up the ripping tool. They went back in with the tools but did not tag cement at 3400 but at 3700. The plug dropped Petroset bulled headed a 300 foot plug and displace it to 3400. Shut well in over night will check top of cement tomorrow. |

Bishop Well Service and Petreset Cementing Service are the contractors on location. Petroset Cementing went in the well with their ripping tool and tagged cement at 3209. They pulled up to 2675 and ripped the 4.5 casing and then pulled 9 jts of 4.5 casing. Petroset Cementing established circulation with 15 barrels of fresh water and pumped the Big Lime plug from 2422 to 2222 with 50 sacks of class A cement 2% gel and displaced with 55 barrels of water through 4.5 casing. Bishop well Service pulled 4.5 casing to 591. Petroset Cementing established circulation with 10 barrels of water and pumped the Brea plug from 591 to 232 with 65 sacks of class A cement with 2% gel. Bishop Well Service pulled 4.5 casing to 200. Petroset Cementing pumped a 200 plug from 200 to ground level. Bishop Well service pulled all 4.5 casing and Petroset Cementing topped well of. Will check cement top on 2/2/2017. |

I found the fluid inside the 8.625 casing to be free of any visual indication of leakage. The cement top was found at 10 down the surface casing utilizing a tape measure. Daley Excavating excavated around the 8.625 casing to a depth of eight (8) feet. Bishop Well services cut off six (6) feet of the surface casing and attached a steel plate with the required information on top of the casing. A 15 piece of two (2) inch coated line pipe was attached to serve as a monument to mark the location. The lined pit was drained by S & H Services and filled in by Daley Excavating. The location and access road was graded to discourage any pooling of water. A Landowner Waiver ( Norton School Board ) was submitted to waive the restoration, removal of flow lines, establishment of vegetative cover and removal of the lease road requirements. No violations identified during this inspection. |
1523750434 | 2016-12-29 | UrbanProduction Wells | No Violations | The Milkovich # 1 well was scheduled for plugging and abandonment pending fair weather and accomodating the Norton High School schedule ( well on School property ). I was on location to observe the de-construction and removal of the tank battery and conduct a pre-plugging evaluation. The well was operated as a plunger lift well with 1.5 ( 3736 ) tubing inside 4.5 ( 3971 ) production casing. The 8.625 ( 336 ) surface casing valve was opened to atmosphere and rapidly blew down to 0 psi. An opening pressure of the 8.625 annulus was not available due to low pressure. The 4.5 casing had 300 psi and the 1.5 tubing gauge registered 340 psi. The tank battery consisted of one 210 barrel stock tank, one poly 35 barrel brine tank and a 2408 hornizontal separator. Sutton Pump and Supply steamed cleaned the tanks and removed them from location. I inspected the areas under the tanks and did not find any contamination. After the plugging evaluation, I had a discussion with Doug Gonzales concerning the plugging procedures. Due to the surrounding residential area and School proximity it was decided to kill , with saltwater, the well prior to plugging. Location and placement methods of the cement plugs was reviewed and approved by me. No violations were identified during this inspection. |

The location of the previous tank battery was included in the foot print of the Field House structure. The plugged well was located was approximately 80 west of the Northwest corner of the Field House. The access utilized for the tank battery and well has been re-developed into parking lot and roadways. Approximately 1000 of flow and sales lines were flushed with water and left in the ground A Landowner waiver ( Form 5 ) requested that the Landowner be allowed to grade, re-establish vegetation, re-develop the area and leave the flow & sales lines. Final Restoration was passed. |
