0294142591 | 2010-08-12 | Drill / Deepen / Reopen | No Violations | Chem Tech was on site with two loads of dust to solidify the two drilling pits. No violations at this time. |
0672637003 | 2010-07-30 | Drill / Deepen / Reopen | No Violations | Appalachian Well Surveys was on site to a run cement bond log and perforate the Clinton. The bond log indicated that the top of cement and bond quality was sufficient to proceed with the frac job scheduled for this afternoon. The drilling pits are still open. |
0722294385 | 2010-06-29 | Drill / Deepen / Reopen | No Violations | Poulson is drilling ahead on air/soap. The pits are fenced, lined and in good condition with sifficient freeboard. |
0948314177 | 2011-01-25 | UrbanProduction Wells | No Violations | The well is in production. Preliminary restoration will be evaluated as soon as the snow cover is gone and the site can be inspected. A recent water well complaint from a resident at 3201 Hametown Road has been resolved. The analytical results of the water collected by MRM do not indicate any oilfield involvement. |
1076212163 | 2010-07-06 | Drill / Deepen / Reopen | No Violations | Poulson Drilling has moved off location. The pits are fenced, lined and in good condition. They have been pumped down and have sufficient freeboard. No violations at this time. |
1097261768 | 2011-05-13 | UrbanProduction Wells | No Violations | The inspection was for preliminary restoration. There are some bare areas that need repaired and reseeded and the required identification was missing. I contacted Tim Levengood and requested that these issues be corrected immediately. |
1110655664 | 2010-08-24 | Urban Drill / Deepen / Reopen | No Violations | The required security fencing is installed and the well is being pumped up. No violations at this time. |
1116802606 | 2018-05-01 | UrbanProduction Wells | No Violations | Access was gained on a asphalt and gravel driveway. The well was operated as a electric pumping well. The wellhead was surrounded with a chain link fence. The fence was locked. The well was not operating during this inspection. All valves were in the open position. A domestic manifold and meter was connected to the 4.5 casing. The tank battery was surrounded with an earthen dike and locked chain link fence. The required I D signage was posted and legible. The tank battery consisted of one (1) 210 barrel stock tank, one (1) 100 barrel brine tank, one (1) 2408 horizontal separator and one (1) meter station. Proper spacing between equipment was observed. The load lines were plugged and locked. No leaks were found. No violations. |
1341664078 | 2010-07-15 | Drill / Deepen / Reopen | No Violations | The drilling pits are still open but are fenced, lined and in good condition. |
1702954155 | 2010-08-16 | Urban Drill / Deepen / Reopen | No Violations | The drilling pits have been solidified and covered over. The affected area has been rough graded. The tank battery has been built and the required security fencing is being installed. No violations at this time. |
1711214410 | 2010-08-03 | Drill / Deepen / Reopen | No Violations | Service rig is running tubing and rods and will set a pumping unit before leaving. The pits are scheduled to be solidified and closed this week. |
1780373710 | 2010-07-13 | Drill / Deepen / Reopen | No Violations | I received a call from Tim Levengood reporting that the cement inside of the 4-1/2 casing covered the interval to be perforated. One of three things may have happened. 1) the cement engineer did not displace with the necessary volume of water. The calculated volume was approximately 62.5 bbl. The operator reported displacing cement with 65 bbl of water. 2) the latchdown plug did not set as designed. The operator reported setting the plug with 450 psi. 3) a hole or split in the 4-1/2 casing diverted some of the displacement volume. Levengood requested permission to use the drilling pit to circulate into while drilling out the cement inside of the 4-1/2. Permission was granted. |
1781263288 | 2010-07-28 | Drill / Deepen / Reopen | No Violations | Universal and Down Hole Drilling were on site to drill out the cement inside of the 4-1/2 casing. Approximately 100 of cement will need to be removed before the well can be perforated and completed. |
