0329255164 2016-10-26 Urban Drill / Deepen / Reopen No Violations I was on location to check the staus of the Ferrise Unit #1 after the previous frac job. J-Well had opened the opened the well after frac and recovered approximately 700 barrels on frac fluids on flow back. All flow back fluids were contained in several 500 barrels tank. I did not identify any leakage from the tanks. The five (5) fresh water frac tanks were still on location and they were found to be leak free. The service rig found that the sand proppant have filled in ~ 100 of the rathole and would have to be removed. No violations.
0547257982 2016-09-22 Urban Drill / Deepen / Reopen No Violations I was on the Ferrise Unit # 1 drill site to witness the pit construction and lining of the pits. Daley Excavating dug the working pit ( ~ 40 X 100 ) into sandy soil ( no water intrusion ) and Poulson Drilling personnel spread approximately 1000 lbs. of Bentonite gel on the sidewalls and floor of the pit prior to lining the pit with a 9 mil liner. Daley constructed a three (3) section blow pit ( ~ 40 X 100 ) and coated the walls and bottom of the pit with Bentonite prior to lining the pit with a 9 mil liner. Poulson Drilling was anticipated to spud the well on 9/26/2016. No violation identified.
0620911914 2016-10-25 UrbanProduction Wells No Violations I observed the fracing of the Ferrise Unit # 1 well by Reliance Well Service. The Clinton sand was broken down at 2035 psi. and Reliance Well Services ( RWS) pumped two (2) acid stage stages. After allowing to soak for 30 minutes RWS started pumping a pad stage at 1609 psi at 26 barrels per minute ( BPM ). During this stage 50 perf balls ( .5 inch diameter ) were injected into the fluid stream and allowed to seat on the perfs. After a 30 minute wait the well was flowed back twice to clear the perfs. The average rate during the stimulation was 25.7 bpm and average treating pressure was 1782 psi. Total fluid volume pumped was 2141 barrels and total sand proppant was 630 sacks. Instaneous shut in pressure was 1600 psi and the five (5) minute shut in pressure was 1382 psi. No violations identified during this inspection.
0699581985 2016-09-28 Urban Drill / Deepen / Reopen No Violations I was on location to witness the Blowout Preventor ( BOP ) test after the cementing of the 8.625 surface casing. Poulson Drilling Corp. ( PDC ) closed the annular BOP on 6.25 collars and pressured up the system to 250 psi. and closed the annular BOP. I had them release the annular BOP to allow the air to flow out of the system. PDC reclosed the annular BOP again and repressurized the system up to 300 psi. This pressure was successfully held for 15 minutes ( 2251 hrs. to 2306 hrs. ) without any pressure decline. The BOP test was passed. There were four (4) full bottles of Nitrogen available on the rig. No violations identified at this inspection.
0700525826 2016-11-21 UrbanProduction Wells No Violations The Operator revised the location of the tank battery from the original submission of the Presite Survey. The location is to be construction back on the wellsite away from any residential structures. No spacing variances will be required. The previous tank battery location required a spacing variance and fugitive odors could be problematic. The revised tank battery is approved.
0889098201 2016-09-27 Urban Drill / Deepen / Reopen No Violations I was on the Ferrise Unit # 1 location to inspection the drilling activities. Poulson Drilling Corp. was the drilling contractor and was drilling a 11.75 bore hole at 389 on a fresh water mud system. The Berea Sandstone was found at the interval of 349 to 360 ( geolograph ). The pits were found to have proper freeboard level. The location was absent of leaks and / or contamination at the time of my inspection. No violations were identified at this time.
1000282322 2016-11-02 Urban Drill / Deepen / Reopen No Violations I re-inspected the Ferrise Unit # 1 to assess the oil release which occurred during swabbing operations on 11/1/2016. The total volume of oil was estimated to be less than 10 gallons of oil. The majority of the oil was contained in the cellar of the well. During my follow up inspection I observed that all the contaminated soil, diapers and peat moss was removed. I asked Mike Ellenis ( Discovery Oil and Gas ) for a manifest for the acceptance and diposal of the material. Mike Ellenis said he would forward a copy. No violations or further measures required at this time.
1428499808 2016-09-29 Urban Drill / Deepen / Reopen No Violations Poulson Drilling Corporation was drilling at 2403 KB on a fluid drilling system. The pits had sufficient freeboard at the time of my inspection. A walk around inspection did not indicate any violations.
1475163746 2016-09-26 Drill / Deepen / Reopen No Violations Polson Drilling drilled a 14.750 hole and ran 77 of 11 conductor casing. Petroset pumped 100 sacks ( 24.2 bbls ) of class A cement 14.7 ppg with 3% CaCl and displaced with 8 barrels of fresh water. There was 6 barrels of cement returned to surface.
1476386711 2016-10-04 Casing String Witnessed No Violations I was on the Ferrise Unit # 1 location to witness the placement and cementing of the 4.5 production casing. Poulson Drilling Corp. (PDC) drilled a 7.875 bore hole from 450 to 4095. PDC conditioned the bore hole for approximately one (1) hour and removed the drill string. PDC lowered 103 joints of 4.5 11.6 lbs. / ft. casing to 4089. The casing was centralized with 12 bow spring centralizers. The casing hardware also included a 4.5 X 18 guide shoe, latch down plug and baffle. Petroset Cementing Services (PCS) cemented the casing with 175 sacks of Class A cement blended with five (5) lbs. of gypsum per sack. The cement slurry was displaced with 63 barrels of fresh water. After the plug landed the pressure was increased to 1500 psi and held for five (5) minutes without any decline. Upon releasing the pressure the float held. The pits had sufficient freeboard volume. No violations identified during this inspection.
1477076854 2016-09-26 Drill / Deepen / Reopen No Violations I was on location to witness the cementing of the conductor casing . Poulson drilled a 14.750 and ran 77 of 11.750 32 lb/ft conductor casing. Petroset etablished circulation and pumped 100 sacks of class A cement with 3% CaCl2 and 40 lb of hulls and displaced with 9 barrels of fresh water.
1477671471 2016-10-28 Urban Drill / Deepen / Reopen No Violations I was on the Ferrise Unit location to check the status of the well after the fracing. J-Well was on location and performing sand pumping operations. Approximately 100 feet of sand was removed from the 4.5 casing rathole. Fluid level was found at 300 feet down from surface. No pressure was detected. J-Well was employing a 500 barreltank to contain the frac load. The tubing ( 1.5) was on location. Location and lease road are solid, but wet from recent rain. No violations.
1483551456 2016-12-29 UrbanProduction Wells No Violations I was on the Ferrise Unit # 1 location to inspect the wellhead and tank battery construction. The wellhead was configured to operate as a Plunger Lift well. The wellhead was free of leaks and contamination. The spacing between the wellhead and tank battery was compliant with spacing regulations. The tank was built with two (2) 140 barrel stock tanks, one (1) poly 50 barrel brine tank, a 2408 horizontal mechanical separator and a meter station. The spacing between tanks and equipment was compliant with regard to the spacing rules. The required lightning arrestor, charcoal filter, pressure relief valve and sealed inspection hatch were present. The required Urban signage was present, but leaning up against the 140 barrel tank. The fencing and landscaping requirement has not been met as of this inspection, but will weather pending ( Mike Ellenis of Discovery Oil and Gas ). A earthen berm surrounds the entire tank battery. The plastic two (2) inch salesline was on location but not buried. No violations were identified during this inspection.
1499006063 2016-09-19 Urban Drill / Deepen / Reopen No Violations I was on the Ferrise Unit # 1 drilling location to witness site construction and the installation the cellar. Daley Excavating was constructing the access road and clearing the brush from the drill site. Daley Excavating dug a hole, at the site of the well stake, 6 X 6 X 5 and lined the hole with 9 mil liner. A 4 X 5 piece of vinyl corrugated culvert was placed, on end, into the excavation and on the liner. Daley Excavating mixed 20 sacks of Sakrete cement and placed in and around the culvert to insure that the cellar was liquid tight. The remaining liner was tucked into the culvert and will be unfurled in the flow ditch to effect a continuous flow from the cellar to the working pit. No violations identified during this inspection.
1505919523 2017-09-20 UrbanProduction Wells Violations Noted I was on location to evaluate the Ferrise Unit # 1 for preliminary restoration designation. The well was operated as a plunger lift well. The wellhead was fenced, locked, screened and landscaped. The annular pop-off manifold was installed and read 10 psi. Maximum allowable pressure at the surface casing seat was 132 psi. The I D signage was Urbanized compliant. I did not identify any leaks or contamination at the wellhead. The tank battery was surrounded by a locked,screened chain link fence and an earthen dike. The landscaping requirement was met. The tank battery was comprised of two (2) 140 barrel stock tanks, one (1) 35 barrel poly brine tank, one (1) 2410 horizontal separator and one (1) 40 X 24 volume bottle. All equipment met spacing requirements. The stock tanks were equipment with lightning arrestor, low pressure relief valve, sealing inspection hatches and charcoal filters. The hatches were closed. The I D signage met the Urbanized requirements. The loadlines were plugged, but not locked. The well and tank battery was accessed on a gravel road. The Urbanized requirement for a gate was not met. In order to bring the Ferrise Unit # 1 well into compliance the Operator shall install a gate on the access road ( 1501:9-9-05 E (2e) and place locks on the load lines ( 1501:9-9-05 E (2d). The Operator shall call Bob Roush ( 330.690.8837 ) once remedial work is completed for re-inspection.
1509367919 2017-10-30 UrbanProduction Wells No Violations A Compliance Notice ( # 1505919523 ) was issued on 9/21/2017 for the lack of a gate on the access road and lack of locks on the load lines. My inspection on 10/30/2017 revealed that the Ferrise Unit # 1 was brought into compliance with the addition of the access gate and locks on the load lines. All issues with the Ferrise Unit # 1 were brought into compliance.
1939653726 2016-10-24 Urban Drill / Deepen / Reopen No Violations Appalachian Well Survey ( AWS ) was on the Ferrise Unit # 1 location to perforate the well in preparation the stimulate the well. AWS made two (2) gun runs to perforate the Clinton sand through the interval 3912 to 3958 with 73 shots ( .39 entry, 23.5 penetration ). The well head had the required 8.625 casing annular pop - off manifold in place. The gauge on the 8.625 casing was open and did not indicate any pressure. There were six (6) 500 barrel tanks on location. Five (5) tanks were filled with fresh water for the frac job and one (1) tank was positioned to receive the flow back and swabbing fluids after frac. Previously, Reliance Well Services dumped 1000 gallons of HCl acid ( 7.5 % ) with two (2) gallons of inhibitor across the Clinton interval after J - Well Well Services swabbed the 4.5 production casing dry. No violations were identified during this inspection.
2014048104 2016-11-01 UrbanProduction Wells No Violations I received notification ( phone call from David Matak, Discovery Oil and Gas ) that while the J - Well service rig was swabbing on the Ferrise Unit # 1 the well flowed on its own and caused a release of crude oil onto the ground. I contacted Fred Romeo ( Emergency Operations Coordinator DOGRM ) to advise him of the release. I arrived on location to assess the amount of the oil release at 1030 hrs. I found that the cellar surrounding the well head had approximately .5 inch of crude oil float on top of water. Diameter of the cellar was four (4) feet. The estimated volume of oil was three (3) to four (4) gallons. A second area of contamination was located 30 feet to the west of the well head and had areal extent of 30 X 30. This area of contamination consisted of the oil floating on rain water. The thickness of the oil could not be accurately measured. An estimated total for the released oil was than ten (10) gallons. I photographed the affected areas and forwarded them to Fred Romeo for his assesment. Mr. Romeo agreed that total release did not appear to meet the notification release volume of 25 gallons for oil. Daley Excavating arrived on location at 1300 hrs. and began to absorb the free oil with oil absorbing diapers. Peat Moss was mixed with the contaminated soil and removed for disposal at Kimbles Landfill, Dover, Ohio. I requested that a manifest copy for the disposal be forwared to me.
2134654756 2016-09-27 Casing String Witnessed No Violations I was on the Ferrise Unit # 1 location to observe the placement and cementing of the 8.625 surface casing. Poulson Drilling Corp. (PDC) was the contractor on the hole and drilled a 10.75 bore hole to 450KB on a fresh water mud system. PDC lowered 10 joints of 8.625 O.D., 23 lbs. / ft. to 425 and circulated for one (1) hour to condition the bore hole. Three (3) bow spring centralizers were utilized on the casing string. Petroset Cementing Services (PCS) connected to the casing and circulated a 10 barrel Bentonite gel slurry prior to cementing. PCS mixed a cement slurry consisting of 145 sacks of Class A cement blended with 3% CaCl. Mix water temperature was 70 F and pH was ~ 7. Displacement was achieved by the use of a plug and seven (7) barrels of fresh water. I observed approximately eight barrels of cement slurry returned to surface. No violations identified during this inspection.
