  • Access using the old lease road off of Boston Mills road (GPS: 41.25644, -81.58224) which is located west of residence through the yard and into wooded area.
  • Access will be pasted 4 BEE Hives along western side of access to the well they will need to be protected.
  • Along the access road there are two Culverts that will need to be protected or replaced before equipment access.
  • This well is equipped with a gas pressure relief pop off (note: The tubing might have been pulled).
  • The Landowner wants the trees that will be cut down for firewood.

1437495975 2014-08-01 Orphan No Violations This inspection was conducted on 8-1-14 to obtain additional information to complete the orphan case file for Complaint #330, orphan well case number 2004-08-021.

I gathered additional information for the site matrix, location sketch, took additional pictures and confirmed GPS coordinates.

  • Matrix scorechanged from 213 to 176 on the revised form.
  • I obtained additional photos of the well area and access.
  • I confirmed the GPS coordinates of the well: 41.25749, -81.58149.
     Note: Aerial images in data base (RBDMS) show this well on property to the north. The well

This follow up inspection was conducted on 07-16-15 to compile and confirm additional information for this orphan case file.

  • Additional pictures of well casing and area around the casing (work area).
  • Access route is off of Boston Mills Road, through yard past three active BEE HIVES, then into wooded area and through wildlife feed area set up by the landowner then into the well site which has numerous small trees (1-3 in diameter). The ground is wet.

The 7.625 OD collar just fits into the casing lip exposed. There is a 7.5 OD to 4.375 OD swage with two - 2.75 OD side ports both side ports were closed.

1445261994 2015-10-09 Orphan No Violations This follow-up Inspection was conducted on 10-09-2015 to gather additional information for this 
orphan well case.

The additional information is as follows: -81.58224). 

This will be through the yard along the western edge of property.  

 operation will be in warmer weather they will have to be protected). Then through wooded area to
 the well GPS 41.25763, -81.58130. The access and work area will need to be cleared for access 
 and a safe working area.
