I informed I knew of no such list, but would inquire with the Bonding Section in the Columbus office. In the interim Moore Well Service supplied some agencies.
0587466031 2013-09-16 Orphan No Violations Meeting at Uniontown office concerning the Rybka well ( 34-153-60429 ) with Tom Hill, Norburt Lowder, Kyle Tharp and Bob Roush. Tom Hill previously informed the Rybkas that they need to become the registered Owners of the well and supply the required Insurance Coverage and Bonding requirements. Tharp and I arrived on location at 1200 hrs. and proceeded to take LEL levels at 6750 Riverview Rd. ( Morgans residence ). Kyle Tharp found the LEL @ 7, after the previous reading had been as low as 0. Mr Morgan had closed the water well vault up tight as to not allow venting. It appears that the cement block is still off-gassing. Checked both abandoned water well on the ^750 Riverview property and found LEL levels to be 0 for both. I visited the Rybka residence @ 1245 hrs. ( 6614 Riverview Road ), to deliver paperwork for proper registration of the ownership of the well, and found no one home. I placed a phone call to Wendy Rybka @ 1337 hrs. no one answered and ask for a return call.
0889522160 2013-09-14 Orphan No Violations At 0755 hrs. I met with Mike Packard ( Valley Fire District ) observe the LEL readings at various spots around the well ( 34-153-60429 ). At the wellhead -0 , at the vent stack ( 41.25272,81.55707 ) - 0 , at 6750 Riverview road water well vault - 0 LEL , at Lahoski residence ( inside of 6582 Riverview Road ) - 0 LEL, at Rybka residence ( 6614 Riverview Road ) - 0 LEL. All readings were conducted by and on instrumentation supplied by Valley Fire District as they had established a baselinehistory.
1326186989 2013-09-25 Orphan No Violations I was on this well to run a downhole camera in the wellbore to determine numerous factors such as casing integrity, casing length, casing seats, points of natural gas entry into the wellbore amongst others. The downhole camera was supplied by DOGRM and operated by Tom Hill and myself. Depth of investigation was limited by length of wireline at approximately 475 down from G.L. . Camera was started in wellbore at 1000 hrs. and we monitored its progress by a Black and White TV at the surface. I observed casing collars @ 25, 50, 65, 115, 160, 185, 215, 245, and a possible hole in the casing @ 142. Casing seats ( hole reduction ) appeared to be at 115 and 245 . Below 245 the wellbore appeared to be openhole ( uncased ) to the deepest point observed approximately 475. I noticed points of Natural Gas entry into the wellbore at 260, 288, 310. Observations below 310 were impeded by gas misting in the wellbore clouding the camera lens and I could not delineate a depth of entry for the gas. The equipment was removed from the well at 1238 hrs. Moore Well Services ran the isolation bladder back into the well hung on 1 pipe and inflated the bladder to seal off the well and reconnect to the vent stack. The Rybka well was static and allowed to continue to vent to atmosphere. I was off location @ 1400 hrs.
1382095019 2013-09-13 Orphan No Violations I received a call from the Valley Fire District Dispatcher @ 0955 hrs. concerning a house located at 6570 Riverview Road having a Gas Odor in the basement. William Twigg ( Dominion / EOG ) was initially called by the Valley Fire District to check their ( Dominion / EOG ) facilities and pipelines in the area. Upon his review Mr. Twigg determined that the stray gas was not from Dominion / EOG sources. Mr. Twigg discovered elevated LEL gas levels and disconnected the Natural Gas service to the residence at 6750 Riverview Road belonging to Russell Mergan. I arrived at 1045hrs. at the Morgan residence and met with Mr. Twigg and Charlie Riedel ( Fire Chief Valley Fire District 330.657.2292 ) to be briefed on the situation. A well location map was constructed of the area in order to determine possible sources. I located 3 wells within .5 mile that were of interest. Located East of the situation were wells # 22669 ( P & A 6/90 ) , #22215 ( P & A 12/85 ) and a Domestic well ( #20317 ) located approx. 2500 Southwest of 6750 Riverview Road. Upon conversations with adjoining Landowners Mr. Lahoski ( 6582 Riverview Road ) and Mrs. Wendy Rybka ( 6614 Riverview Road ) it was discovered that a Gas well had been drilled approx. 70 years ago by Lahoskis Grandfather for a domestic source. I was shown the location of the well, which ( due to subdivision ) lies on the Rybka property. This well had never been registered or permitted with the DOGRM nor its predecessor. I observed that gas was bubbling outside the casing and venting to the atomsphere and appeared to have been doing such for a period of time. I called DOGRM Regional Supervisor Tom Hill was on location @ 1202hrs.. Wendy Rybka relayed that Moore Well Services had previously performed minor ( valve replacement, etc. ) repairs. I contact Jeff Moore @ 1135hrs. and informed him of the situation and he sent Keith Moore to the location ( @ 1221hrs. ). The wells pressure guage registered 38 p.s.I. at the surface. It appe
1696408386 2013-09-18 Orphan No Violations I inspected the Rybka well and venting system during this inspection. I found no evidence of the gas migration around the wellhead. The venting system was still in place and fully functional. In a conversation with Mr. Lahoski ( closest residence to the vent stack ) He has barely had an indication of a gas odor . I delivered the required Ownership papers to the Rybkas.
