0112008005 | 2013-06-24 | Plug / Plug Back | No Violations | I witnessed Petroset set the final two plugs. The rig operator ran tubing to 2,191 to set the Big Lime plug. Petroset established circulation and pumped 75 sacks of Class A cement. The operator displaced the cement with 6 bbls of fresh water. Then the rig operator removed 61 joints to set the surface plug. Petroset established circulation with 6 bbls of fresh water and pumped 130 sacks of Class A cement. I witnessed good cement returned to surface. The rig operator removed the final 14 joints and the cement was topped off at the surface. The cement was static at surface after 20 minutes. A total of 62 joints of 4.5 casing was removed from the well. |
0237031596 | 2015-03-05 | Final Restoration | No Violations | My inspection found the cement was at the surface and the plug had held. No signs of gas coming from the well. I will contact the company to have the casing cut off and the plate installed. No violations found. |
0548792238 | 2011-11-10 | Production Wells | No Violations | No action has been taken to plug or produce the well. |
0618646519 | 2013-06-18 | Plug / Plug Back | No Violations | I witnessed Appalachian Well Surveys bond log the well for Vanguard. The operator tagged the top of the bottom plug at 3,755. The Salina Salt was estimated at 2,830. Appalachian ripped the 4.5 casing at 2,533. The final plugs are scheduled to be completed by 6/24/2013. |
0734337644 | 2013-06-25 | Plug / Plug Back | No Violations | My inspection found the cement static at surface. Vanguard plans to complete the plug job today and begin final restoration of the site; no violations at this time. |
0769172609 | 2015-03-04 | Final Restoration | No Violations | I was on location for Inpsector Skidmore to witness the plugging of this oil and gas well. The well was not showing any signs of gas. Nicks Well Plugging is the contractor on location to plug the well. Nicks tripped 1.5 tubing into the well and tagged the Big Lime Plug at 2190. The well was circulated on for 30 minutes to clear the wellbore of any debris. Nicks pumped 58 sacks (200) of Class A cement on top of the previous Big Lime Plug. Nicks pulled the 1.5 tubing up to 433, broke circulation, and pumped 60 sacks of Class A cement to cover the bottom of the surface casing. Nicks pulled the 1.5 casing up to 200, broke circulation, and pumped 62 sacks of Class A cement. Cement was circulated to the surface. The tubing was removed from the well and the well was topped off with cement. The well is not to be cut off and plate installed until after my inspection on 3/5/2015. |
0931015867 | 2011-07-15 | Production Wells | No Violations | FOLLOW UP TO NOV # 1997318012 An inspection found the well incapable of production and without identification. The rods and tubing were removed. A swage and valve were the only equipment remaining on the production casing.
Post complete identification at the well. Plug or produce the well or apply for temporary inactive status. |
1339705485 | 2012-05-02 | Production Wells | No Violations | My follow-up inspection of this well site found no action has been taken to plug or produce the well. |
1372082472 | 2013-06-12 | Plug / Plug Back | No Violations | I witnessed Petroset set the bottom plug for Vanguard. The top and bottom perforation was at 3,887 and 3,991. Summit Well Service dug and lined the brine pit. The service rig operator ran 1.5 tubing to 3,957. Petroset established circulation with 35 bbls of fresh water and pumped 30 sacks of Class A cement. The operator displaced with approximately 10 bbls of fresh water. Appalachian Well Surveys was scheduled for 6/18/2013 to tag the bottom plug and rip the 4.5 casing. |
1810676408 | 2012-09-20 | Production Wells | Violations Noted | I conducted a production well inspection status check. My inspection found the well equipped with a pumping unit that was overgrown with weeds, grass, and small trees. The tank battery contained one, 210 barrel oil storage tank, one, 50 barrel brine storage tank, a separator, and the proper identification. I noticed no visible spills or leaks. This well shares a tank battery with permit numbers 2-0853. I researched our database and found no production since 2002. I called the Vanguard Office at 330-223-1074 and told them to produce, file for a plugging permit, or file for tempory inactive status. I will perform a follow-up inspection by 10/31/2012. |
1855348683 | 2015-03-02 | Plug / Plug Back | No Violations | I was on location to witness Appalachian Well Service drill out the top plug for Nicks Well Plugging. During my inspection, I found the rig was not on location. I spoke with Nick, who stated the rig was enroute. The well site was cleared of snow. A pit was dug and properly lined. |
