0083257447 2012-07-16 Production Wells No Violations On 7/16/2012, I conducted a production well inspection status check. My inspection found the well was a plunger lift unit (rabbit) that was in good condition. The tank battery was overgrown with weeds and it contained one 100 barrel oil storage tank, with a faded identification sign and one 50 barrel brine storage tank. I reviewed our production database and found no production reported since 2009. Inspection not passed.
0962816233 2016-01-26 Production Wells No Violations I was on location for a status check. During my inspection, I found the well in production. The well is equipped with a plunger lift assembly. The tank battery has a 100 barrel production tank, a 50 barrel brine tank and a mechanical separator. There is a dike around the tanks. An identification sign is posted. The lease road is in good repair.

1462279518 2016-04-28 Production Wells Violations Noted I conducted this inspection to follow-up on a Temporary Restraining Order issued by the Trumbull

--105 requiring Big Sky Energy Incorporated to suspend all oil and gas operations in the State of Ohio. My inspection found the wellhead equipped with a 1.5 plunger lift system. The flow line has been wrapped and taped with insulation and the insulation on the domestic feed has been removed and is lying on the ground. The gate valve on the wellhead is open and flow line valve is closed. This well is not in production at this time. The domestic tap from the casing is open and supplying house gas. The well produces to a horizontal separator to the east and the production chart in the separator was last changed on 2-3-2016 and is not recording production at this time. The well produces to a diked tank battery containing one (1) 100 barrel production tank and one (1) 50 barrel brine tank. The proper ID is posted on the tank ladder. The north and west sides of the 100 barrel tank are covered in oil and it is apparent that the tank has been over-flowed. There are saturated pads around the load line and there is oil stained rock and soil three (3) feet around the perimeter of the tank. Big Sky Energy will be given one (1) week from the issuance date of this notice to perform the following remedial activities: 1) the tanks will need to be steam cleaned and pulled from the battery, 2) impacted stone and soil will be excavated down to clean soil or clay, 3) the contaminated soil will be disposed of at an approved landfill, and, 4) clean soil and stone will be replaced prior to rebuilding the tank battery.

1465231134 2016-06-02 Production Wells No Violations Follow-up to Compliance Notice #1462279518 issued on 5/3/2016 due to pollution and contamination issues found at this tank battery. My inspection found the flow line from the wellhead to the separator closed. The domestic tap at the wellhead was open and a gauge indicated 48psi on the 4.5 production casing. There was no pressure at the separator and the production chart has not been changed since 2/3/2016. An inspection of the tank battery found that none of the remedial activities required by the prior Compliance Notice have been undertaken to address the pollution and contamination at the tank battery. Big Sky Energy Incorporated continues to be in violation of Chapter 1501:9-1-07 of the Ohio Administrative Code and Chapter 1509.22(A) of the Ohio Revised Code.
