1255545773 2016-02-01 Production Wells No Violations I was on location in reference to compliance notice #1751729176. During my inspection, I found the well not in production. The wells 4.5 casing is reduced to a 2 flow line and further reduced to a 1 flow line. There is a 1 ball valve in the closed position. There is no production equipment associated with the well. The lease road is in stable condition. I called and left a message with Schreiner Oil & Gas for a return call. Photos were taken and available upon request.
1463615940 2013-10-21 Preliminary Restoration No Violations I performed a follow-up inspection. The well was not in production. I contacted Mike Schreiner of Schreiner Oil & Gas at 330-323-2277. I informed Mike that a deadline of 1/18/13 was well passed to produce or plug the well. Mike informed me that the company nearly ready to file for bankruptcy and did not have enough money to plug the well. I obtained Mikes email address and sent him photos of the well and flow line from where the tank battery was located. Finally, I told Mike the well needed plugged immediately.

1489556522 2018-05-02 Production Wells No Violations I conducted a compliance notice follow-up for compliance notice 1751729176. During my inspection, I found the well not in production. The wells 4.5 inch casing was reduced to a 2 inch flow line and further reduced to a 1 inch flow line. There was a meter and two regulators on the 1 inch line, and the ball valve was in a closed position. No production equipment was associated with the well. The lease road was stable. There have been no changes to the well site since the last inspection conducted on 1/11/18.
1751729176 2012-11-26 Production Wells Violations Noted I conducted a production well inspection status check. My inspection found a shut-in 4 swab well tied to a domestic meter. I researched our database and found no production since 1988. There was no existing tank battery; only three pipes sticking out of the ground. I took photos of the well head and pipes. I called Schreiner Oil & Gas at 330-323-2277. They thought the well was plugged in 1988 and will research their paperwork. I told Schreiner Oil & Gas Inc., I was issuing a compliance notice to plug the well and to file for a permit by 1/18/2013.

2063654735 2019-02-21 Production Wells No Violations I conducted a follow-up inspection for Chiefs Order #2018-234 requiring Schreiner Oil & Gas Inc. to plug and abandon the Lenyo #1 within 30 days of receipt of the order issued on 8/23/2018. At the time of this inspection, Schreiner Oil & Gas Inc. has not properly plugged and abandoned the Lenyo #1 as required by Chiefs Order #2018-234. The well was located in a non-active farm field and surrounded by steel posts. The well consists of a 4.5 inch casing swaged into a 2 inch flow line with an open ball valve. The 2 inch flow line was reduced to a 1 inch line and with two regulators and a meter. The valve on the 1 inch line was closed and the pressure on the gauged line had a shut-in pressure of 810 psi. There was no production equipment associated with this well. No identification was posted at the well. Photos are available upon request. As of 2/21/2019, Schreiner Oil & Gas Inc. has not appealed the Order to plug and abandon the Lenyo #1, and is in material breach of Chiefs Order #2018-234.
2084715466 2019-01-17 Production Wells No Violations Follow-up to Chiefs Order #2018-234 requiring Schreiner Oil & Gas Inc. to plug and abandon the Lenyo #1 within 30 days of receipt of the order issued on 8/23/2018. My inspection today found that Schreiner Oil & Gas Inc. has not properly plugged and abandoned the Lenyo #1 as required by Chiefs Order #2018-234. The wellhead is located in a bean field and surrounded by four (4) steel, I-beam posts. The Lenyo #1 wellhead consists of 4.5 production casing equipped with a 4 X 2 reducer and a two (2) inch flow line and open ball valve. The flow line is reduced to a one (1) inch line and is equipped with two (2) regulators and a domestic house meter. The valve on the domestic supply is currently closed and a gauge at the wellhead indicates a shut-in pressure of 820psi. There is no identification posted at the wellhead, and I found no surface production equipment associated with the Lenyo #1. Photos are available upon request. As of 1/17/2019, Schreiner Oil & Gas Inc. has not appealed the Order to plug and abandon the Lenyo #1, and is in material breach of Chiefs Order #2018-234.
