0087156932 2014-04-14 Administrative Inspection No Violations I performed an administrative inspection in reference to Compliance #1372354586. My inspection of our database found Energy Exploration has failed to file for a plugging permit. I called the office of Energy Exploration and left a message. I instructed Energy Exploration to immediately file for a plugging permit.
0171424518 2015-10-05 Production Wells No Violations I was on location for a follow-up inspection in reference to Chiefs Order 2015-142 and Compliance Notice No. 1372354586. During my inspection, I found the corn crops around the well have not been removed.
0839696669 2017-09-19 Final Restoration No Violations I was on location for a final restoration inspection. I found the well site graded. Agricultural crops and vegetation are well established. Pits were not used during the plugging operations. There were no production equipment and underground pipelines associated with the well. The access road has been restored. Final restoration passed.
1372354586 2013-06-27 Production Wells Violations Noted My inspection found the well not in production. The well consisted of a 4 x 2 swage with a closed ball valve that is capped. A wind vane was place over the wellhead as a marker. There was no production equipment associated with this well. The landowner was using the lease road to park semi-trailers. There was no identification posted. I researched our database and found zero production report since 2001. I called John Ross at 330-637-4955. John claimed to be in talks with the landowner concerning a possible transfer of ownership. I told John that a compliance notice would be issued to file for a plugging permit and no identification. The permit must be issued by August 2, 2013. The well will be plugged within 180 days if not produced.
1398362918 2014-04-22 Production Wells No Violations I was on location to perform a follow-up inspection in reference to Compliance #1372354586. My inspection found the well not in production. There was a 4 piece of PVC pipe marking the wells location. I contacted ODNR in Columbus on 4/21/2014 and found that Energy Exploration had not filed for a plugging permit. I took photos of the well. There was no production equipment associated with the well.
1435680531 2015-06-29 Production Wells No Violations I was on location for a follow-up inspection in reference to Chiefs Order 2015-142 and Compliance Notice No. 1372354586. During my inspection, I found the well has not been plugged. The well is surrounded by corn crops. According to our RBDMS database, Energy Exploration has obtained a plugging permit dated 4/10/2015. I called and spoke with Energy Exploration about plugging the well. Energy Exploration stated J Well Service Inc. has been contracted to plug the wells. Next, I called and spoke with Jim Adams, owner of J Well Service Inc. Mr. Adams stated Energy Exploration gave him the approval to plug the well on 6/15/2015. I asked Mr. Adams to meet with me and the landowner, Michael Roscoe, to discuss plugging the well. Mr. Roscoe requested that Mr. Adams wait until late September to early October after the soy beans have been removed. Also, Mr. Roscoe stated plugging the well at this time with the current weather conditions could potentially damage crops. I told Mr. Roscoe and Mr. Adams the well shall be plugged immediately upon removal of the crops.
1447685702 2015-11-05 Plug / Plug Back No Violations I was on location to witness J Well plug the well for Energy Exploration. J Well ripped the casing at 2,900. The rig operator pulled one joint to set the Big Lime plug. Plug #2 was set from 2,900 to 2,600. Petroset rigged up and established circulation with 10 barrels of fresh water. Petroset mixed and pumped 75 sacks of Class A cement. The cement was displaced to 2,600 with 41 barrels of fresh water. Next, the rig operator pulled 73 joints of 4.5 casing to set the Berea Sandstone plug. Plug #3 was set from 375 to surface. Petroset rigged up and established circulation with one barrel of fresh water. Petroset mixed and pumped 105 sacks of Class A cement. I witnessed good cement returned to surface. The rig operator pulled the final 11 joints of casing and the well was topped off with cement. The cement held static after 20 minutes. A total of 85 joints of 4.5 casing was removed from the well.
1476133789 2015-11-04 Plug / Plug Back No Violations I was on location to witness J Well plug the well for Energy Exploration. Petroset was the cement contractor. J Well ran 1.5 tubing to 4,335 to set the bottom plug. Petroset rigged up and established circulation with five barrels of fresh water. Petroset mixed and pumped 25 sacks of Class A cement. The cement was displaced to 4,035 with 11 barrels of fresh water. Next, J Well removed all of the tubing and shut in the well for the night.
1532572502 2014-05-16 Administrative Inspection No Violations I performed a follow-up administrative inspection in reference to compliance notice #1372354586. On 5/2/2014, I spoke with John Ross concerning the well. I instructed John to file for a plugging permit immediately. I will continue to perform follow-up inspection.
1989514343 2015-11-03 Plug / Plug Back No Violations I was on location to witness J Well plug the well for Energy Exploration. During my inspection, I found J Well on location rigging up. Next, the rig operator tagged the bottom of the well at 4,375. J Well will run 1.5 tubing into the well tomorrow morning (11/4/2015) to set the bottom plug.
