The well owner/operator shall submit a written plugging schedule to the inspectors office at 3575 Forest Lake Drive, Suite 150, Uniontown, Ohio 44685 within 30 days and plug the well within 180 days. The access road is not assessable due to overgrown vegetation, shrubs and trees. The RBDMS database shows Energy Exploration has filed zero annual production from 2003 to 2014 and has failed to file annual production for 2015 and 2016. At this time, Energy Exploration has not filed for a plugging permit and has not submitted a written plugging plan. I was unable to leave a message for a return call with Energy Exploration due to the answering machine being full. Photos were taken and are available upon request. |

Vegetative maintenance has been conducted at the well and tank battery locations. All vegetation has been cleared from the lease access as well. I contacted Energy Exploration & Development and spoke with well tender Carl Fyfe, regarding the plugging of this well. Mr. Fyfe stated that Energy Exploration & Development is going to be putting this well back into production. |

The well had not been plugged, and was equipped with a plunger lift assembly and not in production, at the time of this inspection. The gate valve on the tubing was closed. The casing valve on the 4.5 inch casing was also closed. The well was covered with extensive vegetation. The diked tank battery was comprised of one 100 BBL tank and a mechanical separator. Valves on the separator were closed and no chart was present inside the chart box. The tank battery was also covered with excessive vegetation. Identification was posted on the separator. Lease access to the well site was no longer usable due to overgrown vegetation, shrubs and trees. At this time, Energy Exploration has not filed for a plugging permit and has not submitted a written plugging plan. I contacted Energy Exploration & Development and left a message regarding the plugging of this well. |
