The well was equipped with a plunger lift assembly and the annulus and flow line valves were open. There was a domestic gas supply plumbed off of the wellhead with an open 2 inch ball valve. There was still a small gas leak coming from the tubing packing. The diked tank battery has four (4) storage tanks. There is approximately 6 to 8 inches of standing water within the entire diked tank battery. Due to the standing water in the dike, I was unable to see if the oil impacted soil and vegetation was cleaned up. There are two (2) separators outside of the dike. Valves on the separator were open. The tank battery was shared with permit #2-2236 and #2-1702. A new identification sign has been stenciled onto the right door of the separator with the owner name listed as Vortex Energy, LLC. Signage was also added at the well location. The permit #2-2237 was stenciled on the separator, which is incorrect for this well. This separator was shared with permit # 2-2236. The access was stable. |

There is domestic line off of the 4.5 inch casing. The tank battery consists of two, 100 BBL production tanks, two, 50 BBL brine tanks. The last date on the chart was 2/29/16 and showed production. There is pooled oil within the tank battery covering half of the inside of the tank battery. Absorbant pads have been placed on top of the pooled oil. A dike surrounds the tanks but not the mechanical separators. The well shares the tank battery with permits 21702 and 22236. Identification signs are posted on the separators but not at the wellhead. The lease road is stable and driveable. The following items need addressed:
1) Clean-up and remove all of the oil in the tank battery and properly dispose of the impacted soil. 2) Add signage to the wellhead. 3) Find and repair the leak in the tank battery. 4) The well needs shut in immediately per the temporary restraining order issued by the Trumbull County Court of Common Pleas, dated 4/11/2016. |

Two heater treater separators were also present and had been installed outside of the diked tank battery. Valves on the separator were open, and the chart on the sales line showed production. The oil impacted soil within the diked battery could not be seen due to the standing frozen water within the battery, and I could not observe if cleanup efforts have been made. One of the heater tubes on the 100 BBL production tank was also covered in frozen ice and I could not see if the leak was still present. Identification signs have been stenciled onto the right door of the separator with the current owner information. The access road was stable. The tank battery was shared with permits 2-2235 and 2-1702. |
