1303905741 | 2015-07-29 | Production Wells | No Violations | On location at 9:00 am to observe continuing oil spill clean-up and remediation. During my inspection, I found Mr. Bob Barr Sr. on location discussing further remediation with Federal EPAs Mr. Stephen Wolfe. Next, Mr. Barr left the well site to continue cleaning along the Big Run Creek in Pennsylvania. Meyers was removing pooled water and oil from trench #1. Additional lined containers arrived on location from IWC. Meyer began scraping the top 6 of impacted top soil between the wire fence and tank battery. The area scraped measured 45 long x 20 wide. Meyers decided to dig two, 2 wide x 2 deep x 15 long trenches between the fence and tank battery to capture residual oil from the spill. After 3.5 hours, I observed no oil or water entering the two trenches. Full containers were taken to IWC to be tested for disposal at a proper landfill. 1437777090 | 2015-07-24 | Production Wells | Violations Noted | I was notified at 10:21 A.M by Terry Whitehair out of the Uniontown office that there was a spill that made it into a creek at this well location. I arrived on location at 11:00 A.M to find the Trumbull County Hazmat crew along with Brookfield fire were on location installing absorbent pads and booms in the Big Run Creek to absorb the oil that had made it into the creek. My inspection found the heating tube on the 100 BBL production tank had rusted through and caused the leak. The 15 BBL of oil that has escaped the tank had made its way 100 to the Big Run Creek and flowed the 500 in the creek before it made it into Pennsylvania. The pasture between the tank battery and the creek was saturated with oil. The pasture that was affected was 100 by 45. The owner of Big Sky, Bob Barr was on location. KDA had a truck on location and was in the process of emptying the 100 BBL production tank, the 50 BBL brine tank, and the puddles of oil in the pasture that could be removed. The company is going to have the tanks removed and the oil saturated soil will be removed as well. The tank battery was not equipped with a proper dike. The well has been shut in until the necessary repairs have been made. |

Myers Well Service had disassembled the tank battery. The 100 barrel and 50 barrel tanks were placed into a newly constructed dike with a liner. A 2 deep trench dug yesterday (7/27/2015) by Myers to cut-off the source of the oil spill contained approximately 4 of water and oil. Myers used their super vac truck to remove all of the fluids from the trench. Next, two roll off containers with liners were delivered to the site by Wolford. Myers began removing impacted soil starting at the tank battery location. Myers dug a hole where the 100 barrel tank was located measuring 3 x 3 x 12 deep. A pool of water and oil immediately seeped into the hole measuring 2 deep. The water and oil was removed and Myers continued to remove all of the impacted material working their way towards the Big Run Creek. A total of 6 roll-off containers were filled and removed by Wolford. The containers were taken to IWC for testing prior to disposal. Photos were taken and are available upon request. Follow-up inspection will continue. |

During my inspection, I found Big Sky Energy had contracted Meyers Well Service to perform the remediation of the oil spill. Next, I attended a morning meeting with the Northeast Ohio Regional Manager Tom Hill, ODNR Inspector Kevin Perry, Federal EPA, Ohio EPA, PA EPA, Brookfield Township Fire Department and Sharon PA Fire Department in Sharon, Pennsylvania where the Big Run Creek enters the Shenango River. Mr. Stephen Wolfe briefed everyone that 95% of the oil in the Big Run Creek and Shenango River was remediated. Mr. Wolfe stated completing all remediation by 7/28/2015. Afterward, I returned to the Big Sky well site. Meyers began digging a 2 wide x 2 deep x 20 long trench #1 within 3 of a wire fence cutting off the oil spill source to the creek from the tank battery. I observed oil entering from the west side of the trench. Meyers used a super vac truck to remove water and oil from the trench. Meyer continued to remove impacted material between the trench and creek. All impacted material was placed in line containers supplied by IWC. Meyer anchored four 4 soft boom in the creek. Follow-up inspection will continue. |

The Federal Environmental Protection Agency is on location instructing Myers on how to perform the clean up of the Big Run Creek bed that was contaminated with oil. |
