0061111419 2015-04-22 Production Wells No Violations I was on location for a follow-up inspection. During my inspection, I found the well still shut-in. The two inch flow line was not repaired. The quart of oil was cleaned and removed. I called Don Gooseman, field supervisor for Big Sky. Mr. Gooseman stated the well tender was working on another project and that the flow line would be repaired next. I informed Mr. Gooseman that another follow-up inspection will be conducted by 4/27/2015.
0063833063 2014-10-31 Production Wells No Violations I was on location for a follow-up inspection in reference to Compliance Notice No. 1412081362. During my inspection, I found another pool of oil between the production tank and brine tank. The oil spill was estimated at 3 gallons. I noticed locks and plugs on the load lines at the brine tank and production tank. I could not determine the source of the oil leak. The dump line from the separator to the production tank had not been repaired. Upon further inspection, I found a bubbling leak next to the flow line at the well. There was a water filled hole with a film of oil. All flow line ball valve were closed indicating the well was shut--up inspection would be conducted on 11/3/2014.
0254967616 2016-04-18 Production Wells Violations Noted I conducted this inspection to follow---105 requiring Big Sky Energy Incorporated to suspend all oil and gas operations in the State of Ohio. My inspection found the wellhead equipped with a 1.5 plunger lift system. The gate valve and flow line valve at the wellhead are open and the domestic tap is open. The casing valve is shut and the control timer at the wellhead is set to cycle in 22 hours. A gauge on the flow line indicates 100psi. The well is producing to a horizontal separator and the flow line and sales line valves are open. The 31 day production chart in the separator was changed on 4-4-2016 and is currently recording production. The well produces to a diked tank battery containing one (1) 100 barrel production tank and one (1) 50 barrel brine tank. The ID on the tank is insufficient as the required information is worn off. Impacted soil between the tanks and below the 100 barrel tank is still present and not mitigated as required under Compliance Notice #1412081362 issued on 12-08-2014. There is a line clamp on the transfer line between the tanks. Big Sky Energy will post the required ID signage at the well within one (1) week of the issuance of this notice. Big Sky Energy Incorporated has failed to comply with the above orders to suspend oil and gas operations in the State of Ohio and shall immediately cease the production operations of this well as required by the temporary restraining order issued by the Trumbull County Court of Common Pleas, dated 4/11/2016.
0321965021 2016-04-11 Production Wells No Violations I was on location in reference to complaince notice 1412081362. My inspection found the well to be equipped with a plunger lift unit. There is a domestic supply plumbed into the 4.5 casing. The valve to the domestic supply is closed. The tank battery consist of a 100 BBL production tank and a 50 BBL brine tank. The heat treated separator is outside of the tank battery with the proper spacing. There is a 31 day production chart that was installed on 4/4/2016 inside of the sales meter. The identification sign is faded and needs to be updated. There is oil stained soil and stone inside the dike between the two tanks from a previous spill. The contaminated soil needs removed from inside of the tank battery and the dike needs to be rebuilt as requested by the previous compliance notice.
0328920216 2019-01-31 Production Wells No Violations I conducted an order follow--105 requiring Big Sky Energy to suspend all oil and gas operations in the state of Ohio. At the time of this inspection, the well was equipped with a plunger lift assembly and all valves on the well, including the the domestic line were closed. The well was not in production. The tank battery location was comprised of one 100 BBL production tank, one 50 BBL steel brine tank and a mechanical separator. Valves on the separator were closed. The tank battery was diked and had legible signage. Access to the well locatoin was stable.
0507598566 2014-10-14 Production Wells No Violations I was on location for a follow-up inspection in reference to Compliance Notice No. 1412081362. During my inspection, I found no locks installed on the load lines. The dump line still needed repaired. Also, the annual production statement had not been filed. I called Big Sky and instructed them to finish making repairs and file the production statement. Follow-up inspections will continue.
1108348332 2015-04-27 Production Wells No Violations I was on location for a follow-up inspection. During my inspection, I found the two inch flow line repaired per my request. The hole was filled and graded. The main valve on the well and the flow line ball valve are in the open position.
1157188160 2014-11-03 Production Wells No Violations I was on location for a follow-up inspection in reference to Compliance Notice No. 1412081362. During my inspection, I found the small pool of oil removed and peat moss spread between the production and brine tanks. The dump line from the separator to the production tank had not been repaired. The hole dug next to the flow line at the well was still open and filled with water. The leak was still bubbling and there was still a film of oil on the water. The flow line ball valve was in the closed position. I called and spoke with Bobby Barr, Jr. Mr. Barr stated the repair to the flow line would be made sometime this week. I reminded Mr. Barr that the dump line needed repaired and to file annual production statement for 2013. Photos were taken. Follow-up inspections will continue.

I learned that the well tender responsible for the well knew of the spill on 9/23/14. I was not informed until the evening of 9/25/14. I informed Mr. Barr that a Compliance Notice is being issued for failure to prevent well operation causing pollution and contamination, failure to keep dike free from oil, fa

1412087738 2014-09-29 Production Wells No Violations I was on location for a follow-up inspection in reference to Compliance Notice No. 1412081362. During my inspection, I found all of the oil was removed, all impacted soil and materials was removed, and the dike had been repaired. A plug was installed on the 2 load line at the brine tank. The stenciled identification sign was fixed. Bobby Barr, Jr. arrived on location during my inspection. Bobby indicated the dump line from the separator to the production tank was damaged while cleaning the site. I informed Bobby to repair the dump line, install locks on the load lines, and file the annual production statement for 2013. Follow-up inspections will continue until all compliance issue are completed.
1431442629 2015-04-18 Production Wells No Violations I received a call from the landowner at 4:00 pm concerning a leak at the well that with blowing gas. I informed the landowner that I would respond immediately to assess the situation. Next, I called and left a message with Bob Barr to dispatch a well tender or field supervisor to the flow line leak. While in route to the leak, I received a call from the well tender stating he was responding immediately and would meet me on location. When I arrived, I initially observed the well was idle and not blowing gas or oil. Upon further investigation, I found the two inch flow line had blew out of a two inch elbow on the discharge side of the flow line ball valve. The well tender immediately shut-in the well. The blown flow line left a two foot diameter hole in the dirt. The poly, flow line was bent in half. I observed one quart of oil in the hole. I instructed the well tender to keep the well shut-in until the proper repairs can be made to the flow line. Also, I instructed him to clean-up the oil. A follow-up inspection will be conducted by 4/22/2014.
1465581096 2016-06-10 Production Wells No Violations I was on location to assess compliance notice # -2549676165. At the time of my inspection, I found the well not in production. The well information has been filled in on the stenciled identification sign on the 100 bbl storage tank. There are no further changes since the last inspection.
1466079943 2016-06-10 Administrative Inspection No Violations Trumbull County Inspector Rick Skidmore conducted a follow-up inspection in regards to Compliance Notice #-254967616 and informed me that the proper ID has been posted at the tank battery. This Compliance Notice is resolved.

1953049236 2015-10-06 Production Wells No Violations I responded to call from the landowner concerning a flow line break at the well. During my inspection, I found a hole in the side of a 2 black, poly flow line. There was approximately one gallon of oil around the break. Weeds around the well had a thin coating of oil. I called Bob Barr, owner of Big Sky, to respond to the flow line break. Upon arrival, Mr. Barr shut-in the well by closing the main flow line valve on the tubing and the 2 flow line valve. Mr. Barr stated the well would stay shut-in until the flow line was repaired. I instructed Mr. Barr to remove the oil and all impacted weeds and soil. Photos were taken and available upon request. Follow-up inspections will be conducted.
