My inspection found the wellhead equipped with a 1.5 plunger lift system. The gate valve and flow line valve at the wellhead are open and the domestic tap is open. The casing valve is shut and the control timer at the wellhead is set to cycle in 22 hours. A gauge on the flow line indicates 100psi. The well is producing to a horizontal separator and the flow line and sales line valves are open. The 31 day production chart in the separator was changed on 4-4-2016 and is currently recording production. The well produces to a diked tank battery containing one (1) 100 barrel production tank and one (1) 50 barrel brine tank. The ID on the tank is insufficient as the required information is worn off. Impacted soil between the tanks and below the 100 barrel tank is still present and not mitigated as required under Compliance Notice #1412081362 issued on 12-08-2014. There is a line clamp on the transfer line between the tanks. Big Sky Energy will post the required ID signage at the well within one (1) week of the issuance of this notice. Big Sky Energy Incorporated has failed to comply with the above orders to suspend oil and gas operations in the State of Ohio and shall immediately cease the production operations of this well as required by the temporary restraining order issued by the Trumbull County Court of Common Pleas, dated 4/11/2016. |

The heat treated separator is outside of the tank battery with the proper spacing. There is a 31 day production chart that was installed on 4/4/2016 inside of the sales meter. The identification sign is faded and needs to be updated. There is oil stained soil and stone inside the dike between the two tanks from a previous spill. The contaminated soil needs removed from inside of the tank battery and the dike needs to be rebuilt as requested by the previous compliance notice. |

During my inspection, I found all of the oil was removed, all impacted soil and materials was removed, and the dike had been repaired. A plug was installed on the 2 load line at the brine tank. The stenciled identification sign was fixed. Bobby Barr, Jr. arrived on location during my inspection. Bobby indicated the dump line from the separator to the production tank was damaged while cleaning the site. I informed Bobby to repair the dump line, install locks on the load lines, and file the annual production statement for 2013. Follow-up inspections will continue until all compliance issue are completed. |

I informed the landowner that I would respond immediately to assess the situation. Next, I called and left a message with Bob Barr to dispatch a well tender or field supervisor to the flow line leak. While in route to the leak, I received a call from the well tender stating he was responding immediately and would meet me on location. When I arrived, I initially observed the well was idle and not blowing gas or oil. Upon further investigation, I found the two inch flow line had blew out of a two inch elbow on the discharge side of the flow line ball valve. The well tender immediately shut-in the well. The blown flow line left a two foot diameter hole in the dirt. The poly, flow line was bent in half. I observed one quart of oil in the hole. I instructed the well tender to keep the well shut-in until the proper repairs can be made to the flow line. Also, I instructed him to clean-up the oil. A follow-up inspection will be conducted by 4/22/2014. |
