1162616345 | 2019-02-27 | UrbanProduction Wells | No Violations | I conducted an inspection on this well, regarding a call received on the One Call line regarding a gas release from a pop-off valve, at the tank battery separator location. During this inspection, the well was equipped with a plunger lift assembly. The gate valve on the tubing and the valve on the 2 inch flow line were closed. The well was not in production, at the time of this inspection. The well had legible identification and was surrounded by tall, landscape grasses. The tank battery was comprised of one 100 BBL tank, one 50 BBL tank, and a mechanical separator. Valves on the separator were open and a new pop-off valve appeared to be installed on the outlet line. The diked tank battery was fenced with a locked man door. Legible identification was present at the tank battery. The lease access was stable and located off the northwest corner in the church parking lot. I contacted Ohio Valley Energy System Corp and spoke with Rick Little regarding the gas release incident. Mr. Little stated that a new pop-off valve and murphy switch would be installed at the well location to monitor line pressure, prior to entering the Dominion gathering line. At the time of this inspection, some of the additional equipment had not been installed and that was why the well was currently shut-in. |

The tank battery contained one, 100 barrel production tank, one, 50 barrel brine tank, and a separator. It was surrounded by a chain link fence with the gate off the hinges. There were plugs on the load line. The was an identification sign, but the wrong permit number. The lease road was the parking lot of the church. I called Don Nething at 330-283-5758. I told Don to re-install the gate with a lock and put the correct permit number on the identification sign. |
