The well was a plunger lift unit. There were two, domestic gas line on the 4.5 casing at the well. The tank battery consisted of a 100 barrel production tank, a 50 barrel, poly brine tank and a separator. The dike was completely weathered. The separator sat outside the dike. Identification signs were properly posted at the tank battery and well. A plug was installed on the load line. The lease road was in stable condition. The well was placed into production prior to House Bill 278. I called Don Nething of Ohio Valley to build a new dike. I told Don a follow-up inspection would be conducted by 6/2/2014. |

The 2 inch flow line valve was open. There was an active domestic line plumbed off of the 4.5 inch casing with two regulators, two meters and a drip bottle. The production records for this well were current. The tank battery consisted of one 100 BBL tank, one 50 BBL poly tank and a mechanical separator. Valves on the separator were open. The tank battery was diked. Legible signage was located at the tank battery and the well. The lease access was stable. No violations observed. |
