0452164543 2013-11-18 Plug / Plug Back No Violations I witnessed Nicks Well Plugging plug the well for Petrox, Inc. Nick removed 4,437 of 1.5 tubing from the well. Next, the operator pumped 10 bbls of water followed by bull heading 30 sacks of Class A cement with 50 lbs of Hauls for Plug #1. The plug was set from 4,286 to 4,578. Once the plug was in place, the well was shut-in overnight with 2,000 psi.
1001102184 2013-12-16 UrbanProduction Wells No Violations My inspection found all of the production equipment removed. Landowner waiver has been obtained for the lease road. Grading and seeding will be completed in early spring due to the weather.
1077053350 2013-11-19 Plug / Plug Back No Violations I witnessed Nicks Well Plugging plug for Petrox, Inc. Nick ripped the 4.5 casing at 3,343 and pulled 8 joints to set the Big Lime Plug #2. The operator established circulation with 10 bbls of fresh water and pumped 86 sacks of Class A cement. The cement was displaced with 43 bbls of fresh water. Plug #2 was set from 2,704 to 3,004. Next, Nick pulled 64 joints of 4.5 casing to set Plug #3 for the surface casing within the Ohio Shale. The operator established circulation with 15 bbls of fresh water and pumped 125 sacks of Class A cement. I witnessed good cement returned to surface. Plug #3 was set from 411 to surface. The operator pulled the remaining 12 joints and the cement was topped off. The cement was static after 20 minutes. A total of 84 joints of 4.5 casing was removed from the well. All circulated water and cement was pumped into tarp covered half round container.
1314992996 2014-06-24 Final Restoration No Violations I was on location to perform a final restoration. I found during my inspection that all production equipment had been removed. The site was graded. Vegetation was well established. There was no pits used during the plugging of the well. All debris was removed from the site. No landowner waiver for the lease road was required; access to the well and tank battery was by an asphalt driveway owned by the condominium complex. Final Restoration passed.
1451084086 2014-04-18 Plug / Plug Back No Violations My inspection found the well site not graded or seeded. All production equipment was removed from the location. I called Petrox, Inc. and left a message for an approximate date to complete the final restoration.
1679275151 2012-06-14 Production Wells No Violations On 6/14/2012, I completed a production well inspection status check. My inspection found the well was equipped with a pumping unit surrounded by a chain link fence that was in good condition. The tank battery contained two 100 barrel oil storage tanks, one 50 barrel brine storage tank, a separator, and the proper identification was posted. I noticed no visible spills or leaks. The lease road was in good condition and located between two pine trees, close to the entrance to the housing allotment. There are no violations at this time.
