1397136086 2014-02-20 UrbanProduction Wells No Violations I responded to a requested inspection from Rick Little of Ohio Valley concerning the landowner not allowing the well tender on the property to perform maintenance on the well and tank battery. My inspection found the well in production; however, the flow line valve at the plunger lift unit was closed. Two domestic gas lines came off the 4.5 casing. The tank battery consisted of a 100 barrel production tank, a 30 barrel brine tank, and a separator. The dike was weathered and not capable of meeting containment requirements. The was a tree and small shrubs in the dike. A bull plug was installed on the load line. Access to the tank battery was the driveway and parking area for Global Oil & Gas Services. While on location the business/landowner, Rick Glass, arrived on site. I asked the landowner about the situation with not allowing the well tender on location to service the well. Rick Glass stated the well tender was accessing the tank battery illegally because the current location of the tank battery was in the wrong location. Also, he stated the access road should be coming off of McDonald Ave., which is east of the well and tank battery. Rick had safety concerns with the ladder not being properly bolted to the tank and that the bottom of the steps were off the ground; he did not want to be held liable for any injuries that may occur as a result of the current location of the tank battery. Rick stated the entire matter concerning the tank battery and the access road was being handled by his lawyer.
1401464224 2014-05-30 Production Wells No Violations I received a phone call from Rick Skidmore asking me to inspect this well for contamination issues. While on location I spoke with both the landowners and the well tender. The well was serviced the previous day and the company had spread peat sorb around the well to absorb the oil and brine that was caused due to the maintenance performed. The 100 BBL production tank had a thin coat of oil down one side of the tank from unloading the well into the tank. I instructed the crew on location from the company to wipe down the tank and clean up any contamination around the well. I spoke with my supervisor Tom Hill about the issue and he was in contact with the company. A meeting with the company man from Ohio valley was scheduled to discuss issues on location.
1401902503 2014-06-03 UrbanProduction Wells No Violations

The well is a plunger lift system that has been recently worked over. During the work over the operator had placed rig mats down to protect the lawn. Mr. Glass was concerned about the oil spray around the wellhead from the work over and it happening again. Oil spray from that work over has been cleaned up. The well had minor gas leaking from between the 4.5 inch and tubing. The following actions need taken at the well site: The dike around the production equipment needs completed. Install fencing around the tank battery and well. Locks maintained on all man doors, valves and hatches. Install proper support under the ladder to the production tank. Trees and brush trimmed back away from the tank battery. Packing around the tubing needs tightened. During future work over operations a liner will be placed under the service rig for spill control. Requested actions need completed by June 27, 2014.

1418317870 2014-12-09 UrbanProduction Wells No Violations I was on location for a follow-up inspection. During my inspection, I found a new dike built. There were new, locked chain link fences with barb wire around the production equipment and well. Identification signs were posted at the tank battery and well. Cement blocks were placed at the base of the ladder on the production tank.
