The well is equipped with a plunger lift assembly. There is a domestic gas line coming off of the wells 4.5 casing. I observed a 6 diameter pool of oil next to the well resulting from a leaking gate valve. The tank battery consists of a 210 barrel production tank, a 50 barrel, poly brine tank and a mechanical separator. There is a dike around the tank battery. An identification sign is posted. There is a plug on the load line. The lease road is in stable condition. I called PAC Drilling and instructed them to repair the main gate valve and remove all of the oil. A follow-up inspection will be conducted. |

2072430432 | 2018-04-09 | UrbanProduction Wells | Violations Noted | At the time of this inspection, the well was equipped with a plunger lift assembly and the annulus and flow line valves were open. There was an active domestic line off of the 4.5 inch casing with two regulators, a drip bottle and meter. The production records for this well were current. It was observed that there was an active leak at the base of the lubricating head. Oil from the leak had run down the wellhead assembly and accumulated at the base of the well. The tank battery consisted of one 210 BBL production tank, one 50 BBL poly brine tank and a mechanical separator. Valves on the separator were open and the chart showed production. The tank battery was diked. The tank battery was full of water and oil/parrafin was floating on top of the water within the dike. There were oil soaked absorbant rags stacked on the separator. The signage was installed at the well and tank battery and the lease access was stable. I contacted PAC Drilling, LLC and spoke with Rusty Hornhill (330) 519-3761, well tender for PAC. He stated that the adjacent two compressors to the tank battery location was the reason for the oil in the battery. A 1 inch poly line was connected to the 210 BBL tank from the compressor, had come loose from the 210 BBL tank causing the oil to spill into the diked battery. Oil absorbant pads were used initially to remove most of the oil when the 1 inch line had become detached from the 210 BBL tank. Per our phone discussion, PAC Drilling needs to:
1) Repair the lubricating head leak at the well and remove the oil impacted soil at the well 2) Remove the oil/water inside the dike and the absorbant pads left from the initial cleanup, and disposed of properly |
