0211697185 2014-06-26 UrbanProduction Wells No Violations I was on location for a status check inspection. I found during my inspection the plunger lift unit in production. The tank battery consisted of a 210 barrel production tank, a 50 barrel poly tank and a separator. A dike surrounded the production equipment. Identification signs were properly posted at the well and production tank. The load line had a bull plug. Locked, chain link fences surrounded the production equipment and well. The lease road was in stable condition.
0413583261 2018-05-11 UrbanProduction Wells Violations Noted At the time of this inspection, the well was equipped with a plunger lift assembly. The annulus and flow line valves were open. There was an identification posted on the chain link fence that surrounds the well, but the fence still needs a lock. The diked tank battery has two storage tanks and a separator. The pick-up line has a plug, but still needs a lock or the handles removed. There was a leaking 2 inch ball valve inside of the separator, which has leaked out onto the ground in front of the separator. There is a build-up of salt and rust on the 2 inch ball valve and the fittings below it. The pick-up lines have plugs, but need locks or the handles remvoed. There was a locked chain link fence around the tank battery. The top of the production tank extend more than 3 foot above the fence and needs modified as required by 1501:9-9--9--e). An identification sign was posted on the fencing, but was missing the required information per 1501:9-9--e). There was a domestic gas supply coming off of the sales line west of the tank battery. There was no landscaping at the tank battery; however, the tank battery and well is located over 1,000 feet from Route 304 on commercial/industrial property. The access road was stable with no gate at the entrance. The access road is the entrance for several commercial/industrial businesses.

At the time of this inspection, the well still needs a lock on the fence man door. The handles have been removed from the pick-up lines. The leaking 2 inch ball valve inside of the separator, has been repaired. The built up salt crystals and absorbant pads inside the separator have not been cleaned up and removed. The impacted soil outside of the separator from the leaking valve still needs removed. There was a locked chain link fence around the tank battery. The top of the production tank extend more than 3 foot above the fence and needs modified as required by 1501:9-9--9--e). An identification sign was posted on the fencing, but was missing the required information per 1501:9-9--e). There has been no landscaping installed around the tank battery.

On 12/13/2016, Northeast Regional Supervisor Tom Hill and I met with Justin Caldwell, co-owner of PAC Drilling, and Rusty Hornhill, well tender for PAC Drilling concerning the missing required urban safety equipment on 31 PAC Drilling wells in Hubbard and Liberty Townships. Mr. Caldwell has been given a document listing the wells with the missing the urban safety equipment. PAC Drilling is to submit a schedule for equipment installation for review and approval by the Division of Oil & Gas.

1891973001 2019-02-04 UrbanProduction Wells No Violations I conducted a compliance notice follow-up for compliance notice 413583261, regarding multiple compliance issues related to urban rule requirements. At the time of this inspection, a lock has been added to the well fence man door. The handles have been removed from the pick-up lines. The leaking 2 inch ball valve inside of the separator, has been repaired. The impacted soil outside of the separator from the leaking valve has been remedied. There was a locked chain link fence around the tank battery. The top of the production tank extend more than 3 foot above the fence and needs modified as required by 1501:9-9-05E(4)(f). The production tank was missing the required urban safety equipment, and needs installed as required by 1501:9-9-05E(4)(b-e). Only a charcoal filter has been installed on the 210 BBL tank. A pressure relief valve and spark arrestor still need to be installed on the 210 BBL production tank. The identification was posted on the fencing and amended required information has been added per 1501:9-9-05E(1)(a-e). There has been no landscaping installed around the tank battery.

PAC needs to: 1) Install urban safety equipment on the production tank as required by 1501:9-9-05E(4)(b-e): Functioning low-pressure relief valve. Tank hatch lids shall have a functioning seal and the hatch shall be secured at all times. Functioning lightning arrestor 2) Install the landscaping at the tank battery location as required per 1501:9-9-05(E)(6)(a-c).
