0528975822 2019-02-27 UrbanProduction Wells No Violations I conducted a compliance notice follow-up for compliance notice 676135366, regarding multiple compliance issues related to urban site requirements. At the time of this inspection, none of the compliance items that were noted in the initial compliance notice have been addressed.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05(E)(4)(f). 3) Repair the fences surrounding the tank battery and well. 4) Secure both fences with locks. 5) Install the require gate with a lock at the access road entrance per 1501:9-9-05(E)(2)(c). 6) Post the proper identification sign on the access road gate per 1501:9-9-05(E)(1)(f). 7) Complete the required landscaping on the south and west sides of the tank battery per 1501:9-9-05(E)(6)(a-c). 8) Install identification at the well location.

0676135366 2018-05-21 UrbanProduction Wells Violations Noted At the time of this inspection, the well was equipped with a plunger lift assembly, and the annulus and flow line valves were open. There was no identification posted at the well. A domestic gas supply comes off of the wellhead with an open 2 inch flow line ball valve, four regulators, a drip tank, and a gas meter. The chain link fence around the well was damaged on the north side. The man-door for the fence was lying on the ground next to the well. The diked tank battery has four storage tanks and two separators. The pick-up lines have plugs and the handles have been removed. The wooden fence around the tank battery has missing boards and damage from falling trees and branches. The man-door on the wooden fence was unsecure with no lock. The required urban safety equipment was installed on both production tanks. The top of the production tanks extend more than 3 foot above the fence. An identification sign was posted on the wooden fence, but was missing the required information per 1501:9-9-05(E)(1)(a-e). There was no landscaping at the tank battery. The fenced tank battery was approximately 90 feet from Route 304. The access road was stable but with no gate at the entrance. The access road gate was not waived during the pre-site inspection. The tank battery is shared with permit #2-3882.
