At the time of this inspection, there was still no lock on the well man door fence and identification still needs added at the well location. The tank valve on the 50 BBL poly tank has been repaired. An identification sign was posted at the tank battery, and has been amended with the required information per 1501:9-9-05E(1)(a-e). The locked chain link fence surrounding the tank battery; has not been replaced with the required 8 foot fence per: 1501:9-9-05E(2). There was no landscaping at the tank battery or well. Place landscaping at the tank battery as required by 1501:9-9-05E(6).

PAC needs to: 1) The existing fence needs replaced with the required 8 foot fence at the tank battery per: 1501:9-9-05E(2). 2) Complete the required landscaping at the tank battery per 1501:9-9-05(E)(6)(a-c). 3) Install a lock and identification at the well location.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Install locks on the pick-up lines or remove the valve handles as required by 1501:9-9-05(E)(3). 3) Install a lock on the fencing at the well as required by 1501:9-9-05(E)(3). -9-05(E)(2) & 1501:9-9-05(E)(4)(f). 5) Place landscaping at the tank battery as required by 1501:9-9-05(E)(6).

1116325590 2015-09-16 UrbanProduction Wells Violations Noted I was on location for an urban status check. During my inspection, I found the well in production. The tank battery consists of a 210 barrel production tank, a 50 barrel, poly brine tank, and a mechanical separator. A dike surrounds the tank battery. Identification signs are posted at the well and tank battery. I observed no plug or lock on the load line. There is no urban safety equipment on the production tank. There are locked, chain link fences around the tank battery and well. The lease road is in stable condition. A compliance notice has been issued to:


Well shall be shut in immediately until all needed equipment is installed. Failure to do so will result in civil penalties. A follow-up inspection will be conducted by 10/09/2015.

My inspection found the well equipped with a plunger lift unit (rabbit) that was in good condition. The tank battery was surrounded by a chain link fence and contained one 210 barrel oil barrel storage tank, one 50 barrel brine storage tank, a separator, and the proper identification. I noticed no visible spills or leaks. The lease road was in good condition. There are no violations at this time.

Install a lock at the well fence as required by 1501:9-9--up lines have plugs and the handles removed. The 210 BBL tank has the required urban safety equipment. There is a locked chain link fence surrounding the tank battery; however, the fencing is 6 foot in height and needs to be 8 feet in height per requirement 1501:9-9--9--9-
