At the time of this inspection, the pick-up lines and tank valves have plugs, but still no locks. Locks need installed on the tank valves and pick-up lines or the handles removed. The identification posted at the tank battery has been amended with the required information per 1501:9-9-05E(1)(a-e). Landscaping has not been installed around the tank battery; however, it was located in a wooded area and not visible from Logan Gate Road. There was no gate installed at the lease access and still needs to be installed.

PAC needs to: 1) Add locks to the tank valves and pick-up lines or remove the handles in the tank battery. 2) Install the required gate with a lock at the access road entrance per 1501:9-9-05(E)(2)(c). 3) Post the proper identification sign on the access road gate per 1501:9-9-05(E)(1)(f). 4) Install the landscaping at the tank battery per 1501:9-9-05E(6)(a-c).

1351180460 2012-10-25 UrbanPreliminary Restoration No Violations I conducted an Urban Preliminary Restoration inspection. My inspection found the well equipped with a plunger lift unit surrounded by a chain link fence. The tank battery was surrounded by a chain link fence, the proper dike, and contained one, 210 barrel oil storage tank with no carbon filter, lightning arrestor, and low pressure relief valve, one, 50 barrel brine storage tank, a separator, and the proper identification sign. I noticed no visible spills or leaks. The lease road was in good condition. I called office for PAC Drilling at 330-874-3781 and request the missing equipment for the storage tank be installed immediately to meet the Ohio Revised Code (ORC) 1501:9-9-5 under urban requirements. I will re-inspect by 11/25/2012.

The well was equipped with a plunger lift unit. It was surrounded by a locked chain link fence with the proper identification sign. The tank battery contained one, 210 barrel production tank, one, 50 barrel brine tank, and a separator. It was surrounded by a locked chain link fence with the proper identification sign. There was a plug on the load line. The required urban safety equipment was installed on the production tank. The dike met containment requirements. The well was in production with a 7 day chart inside the separator. The lease road was in good condition.
1526659484 2018-05-11 UrbanProduction Wells Violations Noted At the time of this inspection, the well was equipped with a plunger lift assembly. The annulus and flow line valves were open. A domestic gas supply was plumbed off of the wellhead with an open ball valve, two regulators, a drip tank and a gas meter. The well was fenced with a locked man door and had legible identification. The diked tank battery has two storage tanks and a separator. The pick-up lines have plugs, but no locks. Locks need installed on the tank valves and pick-up lines or the handles removed. The required urban safety equipment has been installed on the production tank. There was an identification sign posted at the tank battery but needs the required information per 1501:9-9--e). There was a locked chain link fence around the tank battery. There was no landscaping around the tank battery; however, it was located in a wooded area and not visible from Logan Gate Road. The access road was stable. There was no gate installed at the lease access and will need to be installed.
