On 12/13/2016, Northeast Regional Supervisor Tom Hill and I met with Justin Caldwell, co-owner of PAC Drilling, and Rusty Hornhill, well tender for PAC Drilling concerning the missing required urban safety equipment on 31 PAC Drilling wells in Hubbard and Liberty Townships. Mr. Caldwell has been given a document listing the wells with the missing the urban safety equipment. PAC Drilling is to submit a schedule for equipment installation for review and approval by the Division of Oil & Gas.

0056068747 2012-12-11 UrbanProduction Wells No Violations I completed an urban production well inspection. My inspection found the well equipped with a plunger lift unit with the proper identification sign. The tank battery was surrounded by a chain link fence and contained two, 210 barrel oil storage tank, one, 50 barrel brine storage tank, a separator, and an identification sign. The lease road was in great condition. I noticed no visible spills or leaks.

The equipment on the pad consists of the following:



0923106637 2018-05-15 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1838220692. The well was equipped with a plunger lift assembly and the annulus and flow line valves were open. There was an identification posted at the well. There was no fencing around the well; however, the well was located approximately 400 feet from the nearest private residence and over 1,400 feet from Warner Road in a clearing surrounded on three sides by trees. The diked tank battery has three storage tanks and a separator. The pick-up lines are secure with plugs and the handles have been removed from the valves. The required urban safety equipment was missing from the production tank. An identification sign was posted at the tank battery, but was missing the required information per 1501:9-9---3963.

PAC needs to: 1) Post an identification sign to be in compliance with 1501:9-9--e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Install urban safety equipment on the production tank as required by 1501:9-9--e): Activated charcoal filters. Functioning low-pressure relief valve. Tank hatch lids shall have a functioning seal and the hatch shall be secured at all times. Functioning lightning arrestor. 3) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9- 4) Post the proper identification sign on the access road gate per 1501:9-9-

1118238592 2019-02-04 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1838220692, regarding multiple compliance issues related to urban area requirements. At the time of this inspection, there was still no fencing installed around the well. The required urban safety equipment still needs installed on the production tank as required per 1501:9-9-05E(4)(b-e). An identification sign was posted at the tank battery, but was missing the required information per 1501:9-9-05E(1)(a-e). The production tank extends more than 3 foot above the fencing. The access gate was still missing required identification.

PAC needs to: 1) The top of the production tanks extend more than 3 foot above the fence and needs to be remedied per 1501:9-9-05E(4)(f). 2) Update the identification sign posted at the tank battery, with the required information per 1501:9-9-05E(1)(a-e). 3) Install fencing around the wellhead. 4) The required urban safety equipment still needs installed on the production tank as required per 1501:9-9-05E(4)(b-e). 5) Post the proper identification sign on the access road gate per 1501:9-9-05(E)(1)(f).

1838220692 2017-10-11 UrbanProduction Wells Violations Noted I found the well in production on the day of my inspection. There is an identification posted at the well. There is no fencing around the well. The fencing at the well was not waived on the pre--up lines are secure with plugs and the handles have been removed from the valves. The required urban safety equipment is missing from the production tank. An identification sign is posted at the tank battery. The identification sign is missing the required information per 1501:9-9---3963.
