On 12/13/2016, Northeast Regional Supervisor Tom Hill and I met with Justin Caldwell, co-owner of PAC Drilling, and Rusty Hornhill, well tender for PAC Drilling concerning the missing required urban safety equipment on 31 PAC Drilling wells in Hubbard and Liberty Townships. Mr. Caldwell has been given a document listing the wells with the missing the urban safety equipment. PAC Drilling is to submit a schedule for equipment installation for review and approval by the Division of Oil & Gas. | |

At the time of this inspection, the well identification still needs installed. The tank valve on the 50 BBL poly tank has been repaired and no leak was observed, at the time of this inspection. Some of the urban safety equipment has been installed on the 210 BBL tank as required by 1501:9-9-05E(4)(b-e), however a low pressure relief valve is still needed. A plug on the top of the 210 BBL tank also needs installed. It is currently open and allowing gas to vent to atmosphere. The identification posted at the tank battery has been amended with the required information per 1501:9-9-05E(1)(a-e). Fencing has not been installed around the well or tank battery, and was not waived during the pre-site inspection. A lock has been installed on the lease access gate, but required identification still needs installed on the access gate.
PAC needs to: 1) Install the required urban safety equipment per 1501:9-9-05(E)(4)(a-e): Functioning low-pressure relief valve. Tank hatch shall have a functioning seal and the hatch shall be secure at all times. 2) Install a plug on the top of the 210 BBL tank. 3) Post the proper identification sign at the well and on the access road gate per 1501:9-9-05(E)(1)(f). 4) Install the required fencing at the well and tank battery locations. |

The annulus and 2 inch flow line valves were open. There was no identification at the well. The diked tank battery has one 210 BBL tank, one 50 BBL poly tank and a separator. The tank valve on the 50 BBL poly tank was dripping brine at the time of this inspection. The tank valves and pick-up lines have plugs and the handles removed. The 210 BBL tank does not have the required urban safety equipment. Urban safety equipment will need installed on the 210 BBL tank as required by 1501:9-9--e). An identification sign was posted at the tank battery, but was missing the required information per 1501:9-9--e). Both the well and tank battery were not fenced. The fencing was not waived during the pre-site inspection, however the well was surrounded by thick woodlands and approximately 1550 feet away from the nearest occupied dwelling. The tank battery was surrounded by thick woodlands and approximately 1100 feet away from the nearest occupied dwelling. The lease access was stable and shared with well 2-4086. A lock and signage need added to the access gate. I contacted Rusty Hornhill of PAC Drilling about the leaking brine valve on the 50 BBL tank. Mr Hornhill stated that the tank would be emptied and repaired. |

The tank battery consisted of a 210 barrel production tank, a 50 barrel brine, and a separator. The dike met containment requirements. Identification signs were properly posted at the well and tank battery. There was no urban safety equipment at the production tank. The proper plugs were installed on the load lines. The well site was graded and vegetation was well established. The pits were filled and closed. All debris was removed. Drainage and erosion controls were in place. Fencing had been waived because of the isolated and wooded location. I called PAC and instructed them to install the proper urban safety equipment. The lease road was in good, stable condition with a locked gate. |
