The 210 bbl. storage tank is missing the required urban safety equipment. An identification sign is posted at the tank battery. The identification sign is missing the required information per 1501:9-9---site inspection. The tank battery is share with permits 2-3974 and 2-3977.

1109231662 2012-07-30 UrbanProduction Wells No Violations On 7/30/2012, I completed an urban production well inspection status check. My inspection found the well was equipped with a plunger lift unit (rabbit) that was in good condition and located further down Drummand Avenue. The lease road to the well is located on the left, before the small train bridge. The tank battery, shared with permit number 2-3974, was surrounded by a chain link fence and contained two 210 barrel oil storage tanks, one 50 barrel storage tank, a separator, and the proper identification. I noticed no visible spills or leaks. The lease road was in good condition. There are no violations at this time.
1433003646 2019-03-01 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1722656113, regarding multiple compliance issues relating to urban rule requirements. At the time of this inspection, No fencing has been installed around the well and has not been waived during the pre-site meeting. The 210 BBL production tanks have been outfitted with the required urban safety equipment. The identification posted at the tank battery, still needs amended with the required information per 1501:9-9-05E(1)(a-e). There was a locked chain link fence with 3 strand barb wire surrounding the tank battery. The production tank was 3 foot above the top of the fence. There was no landscaping at the tank battery.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05(E)(4)(f). 3) Install the fencing around the well.

1462419685 2018-05-14 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1722656113. At the time of this inspection, the well was equipped with a plunger lift assembly. The annulus and flow line valves were open. An identification sign was posted at the well. There was no fencing around the well. The well was located in an open field surround by woods, and not visible from Drummand Avenue. The diked tank battery has three storage tanks and a separator. The pick-up line has a plug and the ball valve handle has been removed. The 210 BBL storage tank was missing some the required urban safety equipment. An identification sign was posted at the tank battery. The identification sign was missing the required information per 1501:9-9---site inspection. The tank battery was shared with wells 2-3974 and 2-3977.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05(E)(4)(f). 3) Install urban safety equipment on the production tank as required by 1501:9-9-05(E)(4)(b-e): Activated charcoal filters. Functioning lightning arrestor. 4) Install the fencing around the well.
