0041916136 2019-02-28 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1714376331, regarding multiple compliance issues related to urban rule requirements. At the time of this inspection, the wooden fence around the well still needed a lock. The 210 BBL storage tank was still missing the required urban safety equipment. There was an identification sign posted on the fence at the tank battery, but was missing the required information per 1501:9-9-05(E)(1)(a-e). A lock was installed on the tank battery man door fence. The production tank extends more than 3 foot above the top of the fence. The access road was stable with a locked gate at the entrance. Identification still needs installed on the access gate.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Install urban safety equipment on the production tank as required by 1501:9-9-05(E)(4)(b-e): Activated charcoal filters. Functioning low-pressure relief valve. Tank hatch lids shall have a functioning seal and the hatch shall be secured at all times. Functioning lightning arrestor. 3) Post the proper identification sign on the access road gate per 1501:9-9-05(E)(1)(f). 4) Secure the wooden fences at the well with a lock. 5) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05(E)(4)(f).

-up lines have plugs, but no locks. The 210 bbl. storage tank is missing the required urban safety equipment. A deduct meter and 31 day chart dated 29 March 2017 is installed at the separator. There is an identification sign posted on the fence at the tank battery. The identification sign is missing the required information per 1501:9-9-05(E)(1)(a-e). There is a danger, keep out and no smoking sign(s) posted. There is a wooden fence around the tank battery with no lock. The tank battery has woodlands on three (3) sides of the tank battery. The access road is in good repair with a locked gate at the entrance. There is no identification sign on the gate.

1413992855 2014-10-22 UrbanProduction Wells No Violations I was on location for a follow-up inspection. During my inspection, I found a new dike built per my request.
1453921216 2014-07-09 UrbanProduction Wells No Violations I was on location for an urban preliminary restoration. I found during my inspection the well in production with a plunger lift unit. The tank battery consisted of a 100 barrel production tank, a 50 barrel, poly brine tank and a separator. The dike around the production equipment was completely weathered. Identification signs were posted at the well and tank battery. Wooden fences surrounded the production equipment and well. A plug was on the load line. There was no safety equipment on the production tank. The lease road was in stable condition with a locked gate at the entrance. I called PAC Drilling and instructed them to:

Build a new dike around the tank battery. Install the proper urban safety equipment. I will re-inspect the well by 8/15/2014.
