0153628238 2015-08-31 Production Wells No Violations I was on location for a status check. During my inspection, I found the well in production. The well is equipped with a plunger lift assembly. The tank battery consists of a 100 barrel production tank, a 50 barrel, poly brine tank, and a mechanical separator. A dike surrounds the tank battery. Locked, chain link fences surround the well and tank battery. An identification sign is posted. There is a plug on the load line. The lease road is in stable condition.
0277466294 2012-05-21 Production Wells No Violations On 5/21/2012, I completed a follow-up inspection. My inspection found the dike had been repaired. Inspection passed.
1333636956 2012-05-18 Production Wells Violations Noted On 5/18/2012, I conducted a production well inspection status check. My inspection found a ditch dug on the west wall on the dike surrounding the tank batter. I called Don Nething from Ohio Valley Energy Systems at 330-283-1861 to inform him of the violation and the need for an immediated repair. Don informed me the well tender would be sent out that day (05/18/2012) to correct the violation. I will perform a follow-up inspection on 5/21/2012.
1343249567 2012-07-02 Preliminary Restoration No Violations On 7/2/2012, I completed a production well Preliminary Restoration inspection. My inspection found this well equipped with a plunger lift (rabbit) system. The tank battery contains one 100 barrel oil storage tank, with the proper identification posted, one 50 barrel brine tank, and a separator. The pits have been filled and fencing has been placed around the tank battery and well head. The well meets the Ohio Revised Code (ORC) Chapter 1509:9-9-5 requirements. Preliminary Restoration is passed.
