At the time of this inspection, no changes have occurred since my last inspection. The listed items below still need addressed in order for this site to regain compliance.

PAC needs to: 1) Install the barb wire at the top of the existing tank battery fencing. 2) The top of the production tanks extend more than 3 foot above the fence and needs modified as required by 1501:9-9-05E(4)(f). 3) The identification located at the tank battery still needs the required information added per 1501:9-9-05E(1)(a-e). 4) Landscaping at the tank battery still needs installed per 1501:9-9-05 E(6).

1360936482 2013-01-08 UrbanProduction Wells No Violations I responded to a compliant from the Liberty Fire Department for the smell of natural gas. I was asked to investigate all of the wells along Loganway Avenue from Twin Oaks Drive to Route 80. I called Jason Caldwell at 330-340-6238. I instructed him to send their well tender, Rusty, out immediately to inspect all of the production equipment and the wellheads. Also, I told him to walk the sale and flow. My inspection found no leaks or spills and no natural gas smell. All of the production equipment was operating appropriately. Rusty walked the sale and flow line and found no leaks. A total of 6 wells were within the affected area; all belonging to PAC Drilling. The permit numbers for the other wells where 2-4014, 2-4013, 2-4012, 2-3882. I spoke to the Cathy Macchione at the Fire Department and told her of my finding.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05(E)(4)(f). 3) Install urban safety equipment on the production tank as required by 1501:9-9-05(E)(4)(b-e): Activated charcoal filters. Functioning low-pressure relief valve. Tank hatch lids shall have a functioning seal and the hatch shall be secured at all times. Functioning lightning arrestor. 5) Place landscaping on the north side of the tank battery as required by 1501:9-9-05(E)(6).

The well was equipped with a plunger lift assembly, and the annulus and flow line valves were open. There was an identification sign posted on the white plastic fence surrounding the well. The diked tank battery has two storage tanks and a separator. The pick-up lines are secure with the valve handles removed and plugs. The production tank was missing the required urban safety equipment, and needs installed as required by 1501:9-9--e). A locked chain link fence with barb wire surrounds the tank battery. The top of the production tanks extend more than 3 foot above the fence and needs modified as required by 1501:9-9--9--e). There was no landscaping at the tank battery. The tank battery was approximately 80 feet from Loganway Wick Ave. The access road was stable.
