0309546681 2016-02-05 UrbanProduction Wells Violations Noted I was on location for a status check. During my inspection, I found the well in production. There is no identification posted at the tank battery. There are no locks on the load lines. There is no urban safety equipment. I called and spoke with PAC Drilling. I told PAC a compliance notice will be issued for no identification and no locks. Also, I instructed PAC to install the proper safety equipment. I told PAC a follow-up inspection would be completed by February 19, 2016.
0795463271 2018-05-17 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1507734792. At the time of this inspection, the well was equipped with a plunger lift assembly. The annulus and flow line valves were open. The well was fenced with a locked man door and had legible signage. The separator was located near the well and was fence with a lock. The tank battery has two storage tanks. The pick-up lines have plugs and the valve handles have been removed. The 210 BBL storage tank was missing the required urban safety equipment per 1501:9-9--e). The poly storage tank has a collapsed top. The two storage tanks are being held in place by steel cables, 4 inch steel posts and concrete blocks. There was a partial dike around the tank battery; however, the northwest corner of the dike wall has been washed away by the Little Yankee Run. The bend in the Little Yankee Run was approximately within 6 to 8 feet of the northwest corner of the tank battery. The identification sign at the tank battery was attached to the 210 BBL storage tank. The identification sign was missing the required information per 1501:9-9--e). There was no fencing around the tank battery. The fencing at the tank battery was not waived during the pre-site inspection. The tank battery was located approximately 57 feet from Drummond Avenue. No landscaping at the tank battery was present. The access roads to the well and tank battery are stable. The access road gate to the tank battery was not waived during the pre-site inspection. The access road to the well and separator is currently being used as a drive for the campground.
1024673898 2013-10-24 UrbanPreliminary Restoration No Violations My inspection found the well in production. The well was equipped with a plunger lift unit. The separator was in good condition. There were chain link fences surrounding the well and separator with no locks. The tank battery contained a 210 barrel production tank and a 50 barrel brine tank. Both tanks were anchored to concrete blocks. The dike met containment requirements. The proper identification signs were posted at the well and production tank. There was no safety equipment installed on the production tank. The site was graded and vegetation was well established. All debris was removed. Drainage and erosion controls were in place. The lease road was in good working condition. I called the well tender, Rusty, at 330-519-3761. I instructed Rusty to install locks on the fence gates at the well and separator. Also, I told Rusty that safety equipment needed to be installed on the production tank.

We walked over wellsite and lease road areas . We then discussed possible wetlands impact where culverts were plased in lease road . Ed or Alex could not make a determination if wetlands had been affected or not . Ed advised me the Corp or OEPA Needed to be contacted if any stream was to be crossed before any construction occurred to determine possible wetland impact . NOTE: Verbal approval from Alex Kostra to place mud boat in main stream to Myself and Justin Caldwell if Justin applied for tempoary permit for stream crossing .

1507734792 2017-10-11 UrbanProduction Wells Violations Noted I found the well in production on the day of my inspection. There are locked chain link fences around the well and separator. The dike around the tank battery is still washed out in the northwest corner next to Yankee Run River. There is no urban safety equipment on the production tank. The top of the poly storage tank is crushed in and needs replaced. An identification sign is posted; however, it does not meet the requirements of 1501:9-9--up lines are secure with plugs and the handles have been removed from the valves. There is no fencing around the tank battery. The fencing was not waived during the pre-site inspection.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9--e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Install fencing around the tank battery as required by 1501:9-9--9- 3) Install urban safety equipment on the production tank as required by 1501:9-9--e): Activated charcoal filters. Functioning low-pressure relief valve. Tank hatch lids shall have a functioning seal and the hatch shall be secured at all times. Functioning lightning arrestor. 4) Repair the dike around the tank battery per 1501:9-9-05(A)(9).
