1187730347 2017-12-18 UrbanProduction Wells No Violations I was on location to assess compliance notice # -790796538. I found the small gas leak at the well repaired. The handles have been removed from the pick-up lines.

PAC still needs to: Install the urban safety equipment on the production tank. Raise the fencing around the tank battery or install the barbed wire.

1299591206 2011-03-02 Drill / Deepen / Reopen No Violations CASE# 11-001 , Paula Beloit . Inspection of drilling activity , G & H Drilling co. was drilling at 5095 . There were no visible violations at time of inspection . Paula was upset by the noise from the drilling activity , mainly in the night time hours . I advised Paula the rig would be on site for approx. 4-5 days . I also advised Paula that I could not shut-down drilling activitys in the night hours . CASE RESOLVED .

--9-05(E)(1)(a-e). There is no urban safety equipment installed on the production tank. There is no landscaping at the tank battery. The access road is in good repair. The access road gate was waived on the pre-site inspection due to private entrance for the landowner.

The equipment on the pad consists of the following:



1419435506 2018-05-11 UrbanProduction Wells No Violations At the time of this inspection, the well was equipped with a plunger lift assembly. The annulus and flow line valves were open. There was no identification posted at the well, and the fencing around the well still needs installed. The diked tank battery has two storage tanks and a separator. The pick-up lines are secure with the valve handles removed and plugs. The production tank was missing the required urban safety equipment, and needs installed as required by 1501:9-9--e). There was a locked chain link fence surrounding the tank battery. The top of the production tanks extend more than 3 foot above the fence and needs modified as required by 1501:9-9--9--e). No landscaping was installed around the tank battery. The access was stable. The required access gate was not installed and was not waived during the pre-site inspection. Install the required gate with a lock at the access road entrance per 1501:9-9--9-

At the time of this inspection, no identification has yet been posted at the well, and the fencing around the well still needs installed. The production tank was still missing the required urban safety equipment, and needs installed as required by 1501:9-9-05E(4)(b-e). The top of the production tanks extend more than 3 foot above the fence and needs modified as required by 1501:9-9-05E(4)(f). The identification sign was missing the required information per 1501:9-9-05E(1)(a-e). No landscaping was installed around the tank battery. The required access gate was not installed and was not waived during the pre-site inspection, however the access is a shared residential driveway. Install the required gate with a lock at the access road entrance per 1501:9-9-05E(2), and post the proper identification sign on the access road gate per 1501:9-9-05E(1)(f).

PAC needs to: 1) Install the required urban safety equipment on the production tank as required by 1501:9-9-05E(4)(b-e). 2) The top of the production tanks extend more than 3 foot above the fence and needs modified as required by 1501:9-9-05E(4)(f). Raise the fencing around the tanks or install shorter tanks. 3) Amend the identification sign with the required information per 1501:9-9-05E(1)(a-e). 4) Install the landscaping around the tank battery. 5) Install the required gate with a lock at the access road entrance per 1501:9-9-05E(2), and post the proper identification sign on the access road gate per 1501:9-9-05E(1)(f).

1745655594 2015-03-25 UrbanProduction Wells No Violations I was on location for a status check. During my inspection, I found the well in production. The well is equipped with a plunger lift assembly. The tank battery consisted of a 210 barrel production tank, a poly, 50 barrel brine tank and a mechanical separator. There is a dike around the tank battery. Identification signs are properly posted. There was no safety equipment on the production tank. The lease road is in stable condition. I called and spoke with PAC Drilling. I instructed PAC to install the proper urban safety equipment. A follow-up inspection will be conducted by 4/17/2015.
2015050458 2012-12-11 UrbanProduction Wells No Violations My inspection found the well equipped with a plunger lift unit. The tank battery was surrounded by a locked chain link fence. The proper identification signs were posted at the tank and well. It contained one, 210 barrel production tank, one, 50 barrel brine tank, and a separator. The dike was in good condition and capable of proper containment. I noticed no visible spills or leaks. The lease road was in good working condition.
