The equipment on the site consists of the following:



On 12/13/2016, Northeast Regional Supervisor Tom Hill and I met with Justin Caldwell, co-owner of PAC Drilling, and Rusty Hornhill, well tender for PAC Drilling concerning the missing required urban safety equipment on 31 PAC Drilling wells in Hubbard and Liberty Townships. Mr. Caldwell has been given a document listing the wells with the missing the urban safety equipment. PAC Drilling is to submit a schedule for equipment installation for review and approval by the Division of Oil & Gas.

--up lines have plugs, but no locks. The 210 bbl. storage tank is missing the required urban safety equipment. A deduct meter and 31 day chart dated 29 March 2017 is installed at the separator. There is an identification sign posted at the tank battery. The identification sign is missing the required information per 1501:9-9-05(E)(1)(a-e). There is a danger, keep out and no smoking sign(s) posted. The fencing around the tank battery was waived during the pre--site inspection.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Install a locks or remove the handles on the pick-up lines per 1501:9-9-05(E)(3). 3) Install urban safety equipment on the production tank as required by 1501:9-9-05(E)(4)(b-e): Activated charcoal filters. Functioning low-pressure relief valve. Tank hatch lids shall have a functioning seal and the hatch shall be secured at all times. Functioning lightning arrestor.

The equipment on the pad consists of the following:



1528294734 2018-05-17 UrbanProduction Wells Violations Noted At the time of this inspection, the well was equipped with a plunger lift assembly, and the annulus and flow line valves were open. There was identification posted at the well. A domestic gas supply comes off of the wellhead with an open 2 inch flow line ball valve, two regulators, a 4 inch drip tank and a gas meter. The fencing at the well was waived during the pre-site inspection. The diked tank battery has two storage tanks and a separator. The pick-up lines have plugs and the handles removed. The 210 BBL storage tank was missing the required urban safety equipment. There was an identification sign posted at the tank battery, and was missing the required information per 1501:9-9-05(E)(1)(a-e). The fencing around the tank battery was waived during the pre-site inspection. The access road was stable and shared with landowners driveway. The access road gate was waived during the pre-site inspection.

1613269374 2019-02-04 UrbanProduction Wells No Violations I conducted a compliance notice follow-up inspection for compliance notice 1528294734, involving the installation of urban safety equipment on the 210 BBL production tank. At the time of this inspection, The 210 BBL storage tank has had the spark arrestor installed but was still missing the required urban safety equipment, i.e. charcoal filter and pressure relief valve. There was an identification sign posted at the tank battery, and has been amended with the required information per 1501:9-9-05(E)(1)(a-e).

PAC needs to: 1) Install the urban safety equipment on the production tank as required per: 1501:9-9-05E(4)(b-e).
