0290012650 2018-05-17 UrbanProduction Wells No Violations I conducted a compliance notice follow-up for compliance notice 2112323029. The well was equipped with a plunger lift assembly. The annulus and flow line valves were open. The identification sign was posted at the well. There was a locked chain link fence at the well. There was a domestic gas supply plumbed off of the wellhead. The diked tank battery has two storage tanks and a separator. The pick-up lines have plugs, but still need locks. The 210 BBL storage tank has the required urban safety equipment. The identification sign was missing the required information per 1501:9-9-05(E)(1)(a-e). A locked chain link fence surrounds the tank battery. The production tank was 3 foot above the top of the fence. The access road was stable. There was no access road gate and it was not waived during the pre-site inspection. No landscaping has been installed at the well site location, and it was not waved during the pre-site inspection.

PAC needs to: 1) Correct the identification sign to be in compliance with 1501:9-9-05(E)(1)(a-e) at the tank battery with: Well owners address and phone number Property street address or nearest address to the access road entrance. 2) Place landscaping on the south side of the tank battery as required by 1501:9-9-05(E)(6). 3) Install the require gate with a lock at the access road entrance per 1501:9-9-05(E)(2)(c). 4) Post the proper identification sign on the access road gate per 1501:9-9-05(E)(1)(f). 5) Raise the fencing around the tank battery or use low profile tanks to be in compliance with 1501:9-9-05(E)(4)(f). 6) Install locks on all pick-up lines or remove the handles per 1501:9-9-05(E)(3).

I found during my inspection the well equipped with a plunger lift unit that was in production. The tank battery consisted of a 210 barrel production tank, a 50 barrel, poly brine tank and a separator. A dike surrounded the tank battery. Identification signs were posted a the well and tank battery. Locked, chain link fences surrounded the well and production equipment. The proper safety equipment was installed on the production tank. The site was graded. Vegetation was well established. Drainage and erosion controls were in place. All debris and drilling equipment was removed. The pits were closed. The lease road was in excellent condition.

1867996371 2013-05-29 UrbanProduction Wells No Violations My inspection found the well in production. The well was equipped with a plunger lift unit and a domestic gas line. It was surrounded by a chain link fence with no lock. The tank battery contained one, 210 barrel production tank, one, 50 barrel brine tank, and a separator. It was surrounded by a chain link fence with no lock and a dike capable of meeting containment requirements. There was a plug on the load line. An identification sign was posted at the tank battery and wellhead. There was no safety equipment. The lease road was in good condition covered with crushed gravel. I called PAC Drilling at 330-874-3781. I told PAC to install the safety equipment and place locks at both fences.
