Whipstock set 1,023 of 8.625 casing with 6 centralizers. The length of each joint was 42.2. Universal broke circulation, pumped 325 sacks of Class A cement, and displaced. I witnessed 24 barrels of good cement return to surface. The cement held at surface for 20 minutes.

Locked chain link fences surrounded the tank battery and well. The proper identification was posted. Safety equipment was installed. The lease road was in good condition with a locked gate at the entrance. The pits were filled and all debris removed. The production equipment had the correct spacing. The dike surrounding the tank battery was capable of meeting containment requirements.

The tank battery contained one, 210 barrel production tank, one, 50 barrel brine tank, and a vertical separator. It was surrounded by a dike capable of meeting containment requirements. There was a plug on the load line and the proper identification sign posted on the production tank. Bocor has began grading the site. The lease road was in good condition. I will re-inspect by 6/15/2013 to complete the preliminary restoration.
1090245721 2013-06-18 Production Wells No Violations My inspection found the well in production. All production equipped was in place. There was a chain link fence around the tank battery with no lock. The proper identification was posted. The proper safety equipment has been installed. The site was grade, but needed seeded. I called Todd Sutko at 330-506-2531. He stated the restoration of the site should be completed with the next two weeks.
1357151152 2012-12-28 Drill / Deepen / Reopen No Violations I witnessed Universal Well Service cement the 11.75 surface casing. Whipstock drilled the bore hole at 17 to a depth of 650. The casing was set to 617 with four centralizers. Universal used 750 sacks of Type 1 cement with 3% CaCl. The casing was 44.10 at 42 pounds. Eric White, Bocors geologist, called the Berea Sandstone from 335 to 548.
1358259346 2013-01-14 Drill / Deepen / Reopen No Violations I witnessed a plug back job completed by Universal Well Service for Bocor Holdings. The bottom plug was set from 6418 to 7018. Universal broke circulation, pumped 36 barrels of 50/50 cement, and displaced. Whipstock pulled 52 joints of pipe to a depth of 5354. Bocor had Universal set a 100 thief plug from 5254 to 5354. Universal broke circulation, pumped 6 barrels of 50/50 cement, and displaced. A total of 185 sacks of cement was used for both plugs.

1763218789 2017-04-27 Production Wells No Violations I found the well in production on the day of my inspection. The well is equipped with a plunger lift assembly. The ball valve on the tubing is open. The diked tank battery has two (2) storage tanks and a separator. An identification sign is posted at the tank battery. The pick-up line is secure with a plug. There is urban safety equipment of the production tank. Chain link fences surround the well and tank battery. The access road is in good repair. There is a Dominion Master Meter near the access road entrance.

2103087842 2012-12-20 Drill / Deepen / Reopen No Violations I was on location for the cementing of the conductor casing. During the drilling of Hole #1, hard sandstone was encountered at 22. The conductor casing program was shortened to 35 due to the sandstone. I witnessed the cementing of the conductor casing. Universal broke circulation, ran 100 sacks of Type 1 cement, and displaced. Approximately 4 barrels of good cement return to surface.
2113788331 2013-01-15 Drill / Deepen / Reopen No Violations I witnessed the long string cement job performed by Universal Well Service for Bocor. Whipstock set 115 joints of 4.5 casing (10.5 lb/ft) to a depth of 5046. Universal broke circulation with 4 barrels of fresh water, pumped 110 sacks of Unitropic 10-4-2 cement with a density of 14.4 and yield of 1.61, and displaced with 84 barrels of fresh water. The plug landed at 8:40am and held in place.
