0156153062 2013-08-29 Drill / Deepen / Reopen No Violations I witnessed Halliburton cement the surface casing for Brammer Engineer. Nabors M-05 drilled a 17.5 Hole #2 to 294 RKB and set 13.375 casing at 294 RKB. The casing had a weight of 54.5 lbs/ft. and grading of J-55. Halliburton established circulation followed by 50 barrels of 6% Bentonite gel. The operator pumped 150 sacks of Bondcem cement. The cement was displaced to 251 with 39 barrels of fresh water. I witnessed approximately 12 barrel of good cement returned to surface.
0335425719 2013-11-14 Completion Testing No Violations My inspection found Brammer and Halliburton setting up equipment to begin fracturing the well. I spoke with the companyman, Joe. Joe stated there will be 27 frac stages. I noticed no violations at this time.
0394268139 2014-09-22 Final Restoration No Violations I was on location for a final restoration. During my inspection, I found all production equipment removed. All drilling equipment was removed. All debris was removed. Two landowner waivers were submitted for:

1. Well pad will remain. 2. Access road will remain. 3. Electric utility will remain. 4. Waived establishing vegetation coverage of the wellsite and access road. 5. Waived disposal of trees and tree stumps. Final Restoration passed.

0512169958 2014-09-09 Plug / Plug Back No Violations I was on location for the remainder of the plugging of this oil and gas well. The bond log shows the production casing was cemented to surface. The 1800 to 2000 plug was waived by Deputy Chief Dave Clause due to the well production string being cemented to surface. Aztec Well Service ran the 2 7/8 tubing back into the well to 400. Halliburton rigged up, broke circulation, and pumped 40 sacks of Class A cement that scaled at 15.6 ppg to the top of the hole. Cement circulated to surface. The remaining joints of 2 7/8 tubing was removed from the well. The well was topped off with cement. No violations.
0760749505 2013-09-16 Drill / Deepen / Reopen No Violations My inspection found Brammer Engineering reached total depth at 14,140. Brammer was currently tripping out of the hole to run a bond log. I noticed no violations at this time.
0773453421 2013-11-21 Completion Testing No Violations My inspection found Halliburton on fracture stage 6 out of 27 for Brammer Engineering. Brammer was on their third attempt to perforate the well. Halliburton pressure tested the well at 12,000 psi. Next, the pumping rate was brought up to 20 gpm. The down hole pressure (DHP) climbed to 6,000 psi before dropping off; indicating the break pressure for the formation. The operator pumped 2,000 gallons of acid. After the acid reached bottom, the pumping rate was slowly increased to 40 gpm. The DHP climbed with the pumping rate increase. However, once the pumping rate was held static, the DHP continued to climb. The pumping rate was dropped back to 20 gpm to lower the DHP. A second attempt was made by Brammer to increase the pumping rate to 40 gpm and the DHP increase to 10,000 psi. Stage 6 fracture operations were shut down at that point to assess the results. Preliminary results were either a very tight formation or not enough perforations.
1035136806 2013-10-09 Drill / Deepen / Reopen No Violations My inspection found Brammer preparing the well for fracturing operations. Two above ground water storage impounds have been built east of the well pad. I noticed no violations at this time.
1083989169 2014-09-02 Plug / Plug Back No Violations I was on location to witness Brammer plug the well. I found that Brammer was waiting on a permit from Trumbull County to move the service rig from the Jewett Well site.
1112790846 2013-08-22 Drill / Deepen / Reopen No Violations I witnessed Express Energy Services cement the conductor for Brammer Engineering. Express Energy drilled a 26 Hole #1 with an auger rig to 88 and set 20 casing at 86. The casing had a weight of 94 lbs/ft. Hard sandstone was encountered at 25. Express Energy established circulation with 20 bbls of fresh water. The operator pumped 175 sacks of Class A cement, then displaced the cement to 76 with 28 bbls of fresh water. I witnessed approximately 21 bbls of good cement returned to surface.
1123967876 2014-09-04 Plug / Plug Back No Violations I was on location for the plugging of the well. Brammer had J-W Wireline Company on site to run a packer into the tubing prior to removal from the well.
1232785470 2013-11-26 Completion Testing No Violations My inspection found Brammer in-between frac stages. Brammer had completed stage 20 of 27. Halliburton was performing maintenance on their equipment and loading up on sand prior to starting stage 21.
1243764165 2013-10-25 Drill / Deepen / Reopen No Violations My inspection found numerous frac tanks on location in preparation for fracturing around 11/11/2013. There were two above ground fresh water impoundment containers built slightly east of the well pad.
1381250201 2013-09-05 Drill / Deepen / Reopen No Violations I witnessed Halliburton cemented the intermediate casing for Brammer Engineering. Nabors M05 drilled a 12.25 Hole #3 to 2,730 RKB and set 9.625 casing at 2,717 RKB. The casing had a weight of 40 lbs/ft. and graded at N-80. Halliburton established circulation followed by 50 barrels of 6% Bentonite gel. The operator pumped 500 sacks of Bondcem. The cement was displaced with 203 barrels of fresh water. I witnessed approximately 30 barrels of cement returned to surface.
1389879854 2014-01-16 Completion Testing No Violations My inspection found the well not in production and shut-in. The above ground impoundment ponds and piping have been removed. There were no production equipment on location at this time.
1410794393 2014-09-08 Plug / Plug Back No Violations I was on location for the plugging of this oil and gas well. J-W Wireline set a mechanical bridge plug at 5989. The company has Aztec Well Services as the service rig operator on location. Halliburton is the cement contractor on location. The service company tripped in 2 7/8 casing for Halliburton to cement through. Halliburton broke circulation and set a 200 Class A cement plug on

top of the bridge plug using 20 sacks. The cement scaled at 15.6 ppg. Aztec pulled 44 joints of 2 7/8 casing to 4503. Halliburton broke circulation and set 405 of Class A cement that scaled at 15.6 ppg using 41 sacks of cement. Aztec pulled 46 joints of 2 7/8 casing to 3024. Halliburton broke circulation and set 400 of Class A cement that scaled at 15.6 ppg using 41 sacks of cement. The remaining joints of casing were removed and stacked in the derrick. The well was shut in until the well log crew arrives to bond log the well. No violations.

1514577758 2013-06-03 Not Drilled No Violations My inspection found the well pad and lease road construction complete. Drainage ditches ran on both sides of the lease road that were lined with straw matting. Erosion controls were in place using 6 and 10 mulch filled socks. Topsoil stockpiles surrounded by well pad. They were seeded and covered with straw matting. Grass was well established on the stockpiles and the surrounding area. A drainage catch basin was located in the northwest corner of the well pad with a hydrant valve on the effluent side to control flow. A cement soil stabilizer was used on the well pad. The lease road was share with two existing production wells. An identification sign was posted at the entrance of the lease road.
