The equipment on the site consists of the following:



1139557586 2013-11-06 Preliminary Restoration No Violations My inspection found the well site graded. Vegetation was established. The pit was closed and filled. All debris was removed. Drainage and erosion controls were in place. The tank battery consisted of two, 100 barrel production tanks, a 50 barrel brine tank, and a separator. The well shared a tank battery location with permit number 2-3953. The proper identification sign was posted at the well and production tank. The lease road was in good, working condition with crushed limestone.
1376474627 2013-08-14 Drill / Deepen / Reopen No Violations I witnessed PetroSet cemented the surface casing for Cedar Valley. Poulson Drilling drilled a 11 Hole #2 to 317 RKB and set 8.625 casing to 305 RKB. PetroSet established circulation with 2 bbls of fresh water. The operator pumped 100 sacks of Class A Lite cement, followed by 50 sacks of Class A cement. The cement was displaced to 285 with 18 bbls of fresh water. I witnessed approximately 16 bbls of good cement returned to surface.
2077003660 2013-08-28 Drill / Deepen / Reopen No Violations My inspection found the pits closed and graded. Cedar Valley used 75 tons of Kiln Dust to solidify the pits. While on site, I witnessed Appalachian Well Service perforate the well for Cedar Valley. The top perforation was at 3,990 and the bottom was at 4,043.
