0043044183 2013-09-09 Drill / Deepen / Reopen No Violations My inspection found BP running 5.5 casing. The total depth of Hole #4 was 13,990. I found no evidence of weed spraying on the berms; grass was growing.
0243468636 2014-08-18 Plug / Plug Back No Violations I was on location to meet with the company man concerning the start date for plugging the well. I found Brammer waiting for the rig to move from the Roscoe #1 (2-4060). The company man estimated starting at the end of the week.

Nabors M07 rig drilled a 17.5 Hole #2 to 466 RKB (Kelly) and set 13.375 casing to 447 RKB. There were 2 joints of 61# casing and 8 joints of 54# casing with grading of J-55 and 8 centralizers. Halliburton established circulation with fresh water. The operator pumped 50 bbls of fresh water spacer followed by 220 sacks of Bondcem System cement. The plug was released and the cement was displaced to 401 RKB with 62 bbls of fresh water. I witnessed approximately 19 bbls of good cement returned to surface.
0264299512 2013-11-06 Drill / Deepen / Reopen No Violations My inspection found BP was positioning equipment to begin fracturing operations the next day. BP stated there would be 25 stages with an average of 3 per day. I notice no violations at this time.

0362711525 2013-08-22 Drill / Deepen / Reopen No Violations I witnessed Halliburton cemented the intermediate casing for BP. Nabors M07 rig drilled a 12.25 Hole #3 to 2,650 and set 9.625 casing to 2,636. The casing weighed 40 lbs and graded at P-110. Halliburton established circulation with 5 bbls of fresh water followed by 25 bbls of 6% Gel Spacer. The operator pumped 490 sacks of Bondcem System cement and displaced the cement to 2,548 with 190 bbls of water based mud. I witnessed approximately 48 bbls of good cement returned to surface.
1376323209 2013-08-10 Drill / Deepen / Reopen No Violations I witnessed Express Energy Services cement the conductor casing for BP. Express Energy Services Auger Rig drilled a 26 Hole #1 to 84 GL (ground level) and set 20 casing to 75 GL. The casing weighed 94 lb/ft. Express Energy established circulation with 12 bbls of fresh water. The operator pumped 160 sacks of Class A cement, then displaced the cement to 65 with 23.6 bbls of fresh water. I witnessed approximately 10 bbls of good cement returned to surface.
1385127836 2013-11-14 Completion Testing No Violations My inspection found Halliburton on fracture stage 21 of 25 for BP. Break down happened at 6,208 psi. The average pumping rate was 69.7 psi and the average pressure was 6,761 psi. BP dropped a total of 20 balls out of 30 that were loaded. The initial shut-in pressure was 2,864 psi. The backside pressure was static at 71 psi.
1395841876 2014-03-26 Production Wells No Violations My inspection found the well shut-in. There was no production equipment on location. The lease road and well pad were in good repair. Drainage and erosion controls were in place.
1410891054 2014-08-21 Plug / Plug Back No Violations I was on location for a plug job by Brammer Engineering. Mercer Well Service was on location to plug the well for Brammer. Halliburton was the cementing contractor. B-J Services was on location to run the wireline. B-J rigged up and ran a 10,000 psi rated Cast Iron Bridge Plug (CIBP) to 5,600 as the bottom Plug #1. Once the plug was set, Brammer performed a 5,000 psi BP test. The pressure test was static for 15 minutes. Next, Mercer ran 2.875 tubing to 5,600, just above the CIBP. Halliburton rigged up and circulated 110 bbls of fresh water to removed the brine out of the well. Plug #2 was set from 5,600 to 5,400. The operator mixed and pumped 21 sacks of Class A cement. The cement was displaced with 27 bbls of fresh water. Mercer pulled 34 joints of tubing. Plug #3 was set from 4,499 to 4,048. Halliburton established circulation and pumped 46 sacks of Class A cement. The cement was displaced with 21 bbls of fresh water. Next, Mercer pulled 45 joints of tubing. Plug #4 was set from 3,047 to 2,747. Halliburton established circulation and pumped 31 sacks of Class A cement. The cement was displaced with 12 bbls of fresh water. Mercer pulled the remaining joints of tubing. B-J Services rigged up and shot off the 5.5 casing at 1,300. Mercer pulled up on the casing and Brammers company man had Halliburton circulate on the well with fresh water. Afterwards, the well was shut-in.
1782533468 2014-08-28 Plug / Plug Back No Violations I was covering the remainder of the plug job for Rick Skidmore. The company has Aztec Well Services as the service rig operator on location. Halliburton is the cement contractor on location. Aztec set the mechanical bridge plug at 1005. The service company tripped in 2 7/8 casing for Halliburton to cement through. Halliburton broke circulation and set a 200 Class A cement plug on top of the bridge plug using 72 sacks. The cement scaled at 15.6 ppg. Aztec pulled 14 joints of 2 7/8 casing to the bottom of the final plug. Halliburton broke circulation and set 550 of Class A cement to the top of the hole using 199 sacks of cement. Cement was circulated back to surface. Aztec pulled the remaining joints of casing from the well.
2106282450 2014-09-22 Final Restoration No Violations I was on location for a final restoration. During my inspection, I found all production equipment removed. All drilling equipment was removed. All debris was removed. A landowner waiver was submitted for:

1. Well pad will remain. 2. Access road will remain. 3. Electric utility will remain. 4. Waived establishing vegetation coverage of the wellsite and access road. 5. Waived disposal of trees and tree stumps. Final Restoration passed.
