During my inspection I identified violations of OAC 1501-9-9-05 and OAC 1501-9-08 (N). The tank battery was surrounded with an earthen dike and an locked access door. There was only one (1) access door built into the fence. The tank battery consisted of three (3) 210 barrel stock tanks, one (1) 100 barrel brine tank and one (1) horizontal mechanical separator. Identifying signage was legible and on the ground, leaning against the fence. The tanks had the required carbon filters, pressure sealing hatches, pressure relief valves and lightning arrestor. The load lines were plugged, but not locked. The handles were removed. The eastern most 210 barrel and 100 barrel brine tanks did not have plugs in the two (2) inch bungs which allowed natural gas fumes to vent to the atmosphere. There dike and disturbed area around the tank battery was rough graded, but not seeded. Erosional features were present on the barren ground. There was non-essential items in and around the tank battery such as hand rails, a roll of geotex fabric lying along the road, plastic pipes, buckets and assorted fittings. The graveled access road continued to the wellhead. The area of disturbance was rough graded, but was void of vegetation and not stable. Non-essential materials, lumber, pallets and pipes were scattered around the perimeter of the disturbed area. There were erosional features crossing the location from north to south. I found a six (6) to eight (8) inch hole, near the former pit area, where water was observed flowing from and across location. I field tested the effluent from the hole and found the Chlorides level was less than 32 ppm and the pH was 6. The well was completed as an electric pumping well. The wellhead was not identified. The wellhead was fenced, but had gaps in fencing large enough as to offer no security. The access door was not locked. The stuffing box and tubing head were covered with grease and oil. The cel |

The well is currently shut-in with a frac (gate) valve and the annular valve between the 7 intermediate casing and 4.5 production casing is open and quiescent. There are 12-500 barrel frac tanks on the south side of the pad. There is a lined pit on the east side of the pad and the liner has a 15 long tear on the eastern, steep wall of the pit. The fluid level in the pit is 7-8 below the tear in the liner and is not an issue at this time. The silt fence was inspected and no off site sedimentation was found. The location is dry and the gate at the access road is locked. I found no violations at this time. |

At the time of this inspection, the lease access had a locked gate installed with required urban identification posted on the gate. The tank battery had the fencing installed around the production equipment and two man doors were installed and locked. The tank valves and pickup lines on the production tanks were all locked. The eastern most 210 BBL tank and 100 BBL brine tank still needed plugs in the two inch bungs on the top of the tanks installed. The dike surrounding the tank battery had been stabilized with vegetation. The west side of the tank battery fencing had green, privacy slats installed, but not on any other side of the fence. The non-essential items in and around the well and tank battery have been removed from the location. The well was fenced, with green privacy slats installed in the fencing. The gaps in the fence have been remedied and both areas of access to the well had locks. The stuffing box and tubing head were cleaned up and no longer covered in grease and oil. The cellar has been filled with stone and at the time of this inspection no oil or fluid was present. Pop-off valves have been installed on the 2 inch riser pipes plumbed off the annular space between the intermediate casing string and the production casing string, but pressure gauges were still needed. Legible identification with the required information per OAC 1501:9-9-05E(a-i) has been installed at the well and tank battery locations. Landscaping has been installed along the east and south sides of the fenced well location, however some areas at the well location still need established vegetation.
