This wellhead and the Bates #1H are currently equipped with a reducer and ball valve and are protected by jersey barriers. I found no violations at this time. |

Moore Well Services were rigging up to swab the 2.375 tubing with tools in an effort to unload the well. Flow back fluids were to be collected in a 500 barrel tank. There were 16 - 500 barrel frac tanks on location. There two (2) sand transport trucks on location. The pits afforded adequate volume for fluids and a walk around inspection did not reveal any violations during this inspection. |
0369766108 | 2017-10-23 | Completion Testing | No Violations | I was on location to observe various Frac stages of the well. Producers Service Corporation ( PSC) was the stimulation contractor. The lateral was scheduled for 21 stages of treatment. Each stage was 60 in length with five (5) clusters ( 10 shots each, 60 degree orientation ) equally spaced in the stage. The toe prep work was completed on 10/20 17 with the break down pressure reported to be 4600 psi. An acid pad ( 500 gal 15% HCl ) was pumped followed by a slick water pad. Sand concentrations were started at .5 ppg and ranged up to 1.5 ppg. During the stage an increase of the treating pressure reflected issues with sand placement and a sand screen out was encountered. Repeated attempts to flow the sand off the perfs were performed. The downhole pumping rate was insufficient to allow a pump cycle of the perf guns and plug. PCS continued to attempt to clear to casing enough to allow the perf gun to be pumped into position. |

The pressure gauge read zero. The tank battery consisted of three (3) 210 barrel stock tanks and one (1) Gas Production Unit (GPU). An earthen dike surrounded the tank battery. Proper spacing was observed between the production equipment. The preliminary restoration was delayed by weather considerations ( per Cary LeShock, U S Energy ). No violations were identified during this inspection. |

The wellhead is equipped with 2.375 rods and tubing and operating with the aid of an electric powered pumping unit. The stuffing box is equipped with a pumpkin containment system and the containment hood is plumbed to a 55 gallon drum. The casing flow line is equipped with a line heater and the annulus between the 7 intermediate casing and 4.5 production casing is equipped with a 250psi pressure relief valve. Gauges indicate a pressure of 290psi on the tubing and 450psi on the 4.5 production casing. This well produces to a diked tank battery shared with the Bates Unit 1H (PN#24098) and contains three (3) 210 barrel tanks. There is a heat treating, horizontal separator 50 to the north of the tanks and is surrounded by a secondary containment dike. There is a contract pumper currently on location (Tim) and he is transferring production water to a 500 barrel steel tank. I spoke with the U.S. Energy Field Supervisor (Kerry) about the 500 barrel tank and he said that the tank is not permanent and will be removed when they get ahead of the production water. Valves at the wellhead and on the load lines are equipped with locks. Vegetation is well established around the perimeter of the pad and the required identification is posted at the wellhead and at the tank battery. No violations. Preliminary Restoration is approved. |

I spoke with Cary LeShock ( U S Energy )and requested that he included the Township, County and Lot number. The restorative work had been completed, but I could not evaluate the level of compliance due to snow cover. |

There was sufficent pit volume in the blow pit. I requested that the 13.375 to 9.625 annular space be grouted to surface prior to cementing the seven (7) inch casing. A walk around inspection of the air package, mud pump and perimeter did not indicate any violations during this inspection. |
