A gas detector, on the Pason, indicated a show ( 21 units ) below the top of the Lockport Dolomite ( top @ 3312 MD ). There was not any indication of H2S. Erosion controls were in place and effective. No violations identified during this inspection. |

0599845139 | 2018-08-07 | Preliminary Restoration | No Violations | I was on location to check the status of Preliminary Restoration of this horizontal, Clinton Sandstone well. My inspection found the lease road and well pad in stable condition. The well is in production and equipped with 2.375 tubing and rods and operating with the aid of an electric powered pumping unit. Gauges indicate pressures of 340psi on the tubing and 570psi on the 4.5 production casing. The stuffing box is equipped with a pumpkin containment system and the containment hood is plumbed to a 55 gallon drum. The casing flow line is equipped with a line heater and the annulus between the 7 intermediate casing and 4.5 production casing is equipped with a 250psi pressure relief valve. The tank battery is diked and contains three (3) 210 barrel tanks and the separator is located in a separate containment area east of the tanks. All of the load lines are locked. There is an electric pump in the northwest corner of the battery that is plumbed to the load lines, the top of the western tank, and to the top of a 500 barrel frac tank that is sitting outside of containment. The fluid level in the frac tank is 4. Vegetation is well established around the perimeter of the pad and the required identification is posted at the wellhead and at the tank battery. I will speak with U.S. Energy about the status of the 500 barrel tank prior to approving Preliminary Restoration. If the tank will be permanent, secondary containment measures will need to be installed around the tank. No violations. |
0959919975 | 2017-08-14 | Casing String Witnessed | No Violations | I was on location to witness the cementing of the 9.625 surface casing. US Energy drilled a 12.25 hole to a depth of 336 measured from the Kelly bushing (KB +10) and set 26lb/ft., ASTM, A500 casing at a depth of 326 KB with a 7 shoe on bottom. Petroset Cementing rigged up and broke circulation with 30 barrels of freshwater and then mixed and pumped 200lbs of bentonite gel with 40lbs of cottonseed hulls to condition the hole. They then mixed and pumped 31.5 barrels (150 sacks) of Class A cement slurry with 3% calcium chloride. The cement was scaled at 15.6lbs/gallon midway through mixing. The plug was released from the head and displaced to 326 with 26.5 barrels of freshwater and landed in the shoe at 500psi. The pressure was released and the cement held at the surface. I witnessed 5-6 barrels of good cement return to the surface. Mix water pH was 7 and the water temperature was 70 degrees. US Energy will wait 8 hours before breaking off and continuing operations. I found no violations at this time. |

US Energy spudded this well on 8/10/17 and set 56 of 13.375 conductor casing this morning. I retrieved the cement ticket from the doghouse and inspected the casing string below the rig floor. Rig #9 is currently waiting on cement prior to drilling out below the casing shoe. The location was inspected and the pits were found to be lined with minimal fluid in the pits at this time. We met with Cub (US Energy) and he said there were no issues with the conductor casing cement job. He informed me that the rig would be shutting down for the weekend and that they would begin drilling the surface hole early Monday morning (8/14/17). I found no violations at this time. |

The stuffing box is equipped with a pumpkin containment system and the containment hood is plumbed to a 55 gallon drum. The casing flow line is equipped with a line heater and the annulus between the 7 intermediate casing and 4.5 production casing is equipped with a 250psi pressure relief valve. Gauges indicate a pressure of 250psi on the tubing and 500psi on the 4.5 production casing. This well produces to a diked tank battery shared with the Williams Unit 2H (PN#24102) and contains three (3) 210 barrel tanks. There is a heat treating, horizontal separator 50 to the east of the tanks and is surrounded by a secondary containment dike. There is a contract pumper currently on location (Tim) and he is transferring production water to a 500 barrel steel tank. I spoke with the U.S. Energy Field Supervisor (Kerry) about the 500 barrel tank and he said that the tank is not permanent and will be removed when they get ahead of the production water. Valves at the wellhead and on the load lines are equipped with locks. Vegetation is well established around the perimeter of the pad and the required identification is posted at the wellhead and at the tank battery. No violations. Preliminary Restoration is approved. |

I will contact Cary LeShock ( U S Energy ) concerning the installation of the pressure relief valve on the production - intermediate annulus. All the valves on the wellhead were locked. Three 210 barrel stock tanks were on location, but the tank battery construction had not began. No violations were identified during this inspection. |

There was a 55 gallon waste barrel to contain any fluids captured from the leak containment pumpkin on the stuffing box. The 7 to 4.5 annulus had a manifold that included a pop off valve and gauge. The gauge read zero psi. The tank battery consisted of three (3) 210 barrel stock tanks and one (1) Gas Production Unit ( GPU ). An earthen dike surrounded the tank battery. The preliminary restoration has not been completed due to weather considerations. No violations identified during this inspection. |
