0441595015 2018-06-26 Plug / Plug Back No Violations Upon arrival on location, I was accompanied by ODNR Inspector Randy Akins, I witnessed rig crew running tools down well to tag the top of cement plug. The tools sat down at 4841 feet. The cement plug did not stay. They are having 1.5 inch diameter tubing delivered to location. They will run 143 joints of tubing, this will put them at 4787 feet in the well. They will set another bottom plug at this time. The rig crew ran 1.5 inch tubing to the depth of 4787 feet. The cement crew cleared the tubing with 11 barrels of brine water and started to pump a 30 sack Class A cement plug to the depth of 4787 feet. They displaced the cement with 10.9 barrels of brine water to the depth of 4387 feet. The rig crew pulled 1000 feet of 1.5 inch tubing out of the well. They will run in with 1.5 inch tubing in the morning to tag the top of cement plug.
0545309899 2018-06-14 Production Wells No Violations This is a follow up inspection to compliance notice 1572157582 issued on 07/06/2017. On the day of my inspection, I witnessed the well was equipped with 8.625 inch diameter casing and 4.5 inch diameter casing with a 4 inch diameter valve on top. The valves are in the closed position, the well was idle at the time of the inspection. I could not locate any production equipment at the time of the inspection. The production report was up to date but showed no production since 2004. I will contact Enervest to see if applied for plugging permit.

1530102121 2018-06-27 Plug / Plug Back No Violations Upon arrival on location I found the rig running tubing into the wellbore to tag the top of the cement plug. The plug top was at 4625 feet. The plug has the perforations covered, but does not have the height needed to be over the top of perforations. The rig is shutting down for the day and will resume plugging on Thursday. June 28.
1530221418 2018-06-28 Plug / Plug Back No Violations Upon arrival on location, I witnessed the rig crew running 1.5 inch tubing back in well to the depth of 4627 feet. The cement crew cleared the tubing and started pumping a 20 sack, Class A cement plug. This plug will top off the remaining footage required for bottom plug. They displaced with 10.9 barrels of fresh water to the depth of 4361 feet. The rig crew will now trip all the 1.5 inch tubing out of the well. They will run their 4.5 inch tools in the well to rip the 4.5 inch casing at the depth of 3360 feet. They pulled 6 joints (196 feet) to the depth of 3164 feet. Cement crew established circulation on well and started to pump a 60 sack, Class A cement plug. They displaced to the depth of 2964 feet, with 48 barrels of fresh water. Rig crew started to 4.5 inch casing to the depth of 1018 feet. Cement crew established circulation on well and then started to pump a 60 sack, Class A cement plug. They displaced to the depth of 818 feet, with 13 barrels of fresh water. Rig started pulling 4.5 inch casing to the depth of 200 feet. Cement crew established circulation and then started to a 60 sack, Class A cement plug to ground level. They pumped until good cement returns were seen. The rig crew pulled the remaining 4.5 inch casing out of the well.
1530534365 2018-07-02 Plug / Plug Back No Violations Upon arrival on location, I witnessed the rig and all the other equipment had been moved off location. The 8.625 inch diameter casing was cut off below ground level. The small pit was back filled and the location was roughed in. At the time of this inspection, the location and lease road will still need more work done on them before final inspection can be done.
1572157582 2017-07-05 Production Wells Violations Noted

Enervest Operating LLC. needs to submit a written plugging schedule to Inspector Randy Akins at 2207 Reiser Ave. New Philadelphia, Ohio, 44663 within 30 day, per OAC 1501:9-11-05(B), and plug the well within 180 days. The owner/operator will be notified if the plugging schedule is approved or denied.
