0214585807 2015-11-05 Production Wells No Violations Upon arrival on location I met Trish Cartmell to inspect the are of a previous oil spill. The are continues to improve, but there still needs some maintenance. The are of the hard boom needs some cleanup again, The rest of the area is much improved. Trish and I will re-inspect in two weeks and hope to close out the case.
0249264172 2015-10-14 Production Wells No Violations This a follow up inspection of the oil spill. The tank has been replaced, and the dike has been redone and seeded. Where the area has been seeded and mulched the grass is growing. The are looks good. The road ditch still needs to be monitored, and absorbent materials need to be used. The siphon dam on the wetland side can be removed and all the booms can be removed but the hard boom needs to stay in place for now. I was accompanied by Terry Whitehair and Trish Cartmell.

0524517238 2015-12-07 Production Wells No Violations Upon arrival on location I met with Trish Cartmell and Ron with Buckeye Oil. After reviewing the areas of the past oil spill it was decided that the is no need to continue inspections and the case is to be closed. The area shows no signs of further oil seepage. All booms are to be removed and the EPA is to be contacted about removing the boom. The clean up is complete.
1393873640 2015-12-15 Production Wells No Violations This is a follow up inpection of a compliance notice issue on 9/10/2015. The clean up is completed. All of the booms and the absorant pads have been removed and there no signs of any oil in the road ditches or on the area that was impacted.
1441895823 2015-09-10 Production Wells Violations Noted I received a phone call at 8:00 am from Steve Sigler with Buckeye Oil that there tank battery had been vandalized and an oil spill had occurred. I then called Brent Bear Regional Supervisor of the occurrence. Then advised our response team, and talked to Terry Whitehair, and made him aware of the occurrence. The vandals removed a bull plug from the 3 inch load line of the tank. The oil then ran down the lease road to road ditch of Lindentree road. It then continued to a culvert under the road into a wetland area. The total release appears to be 50 bbl of oil. Buckeye Oil is on site and has started clean up. A discussion was held with Steve Sigler with Buckeye OIL and Bob Timmerman with Dart Oil. Two items of concern was how the dike was constructed and that the area is known for vandalism they should have locks on their tanks valves.
1441992289 2015-09-11 Production Wells No Violations Upon arrival on location I found Buckeye Oil crew, Banjo Construction, on site continuing cleanup. Hot water has been used to clean up the dike area and the lease road. Several loads of fresh water has been used to control flood the wetland area to push the oil to controlled site and vacuumed up. There are six sets of four booms to make sure that containment is in place. The flooding has helped move the oil to the containment area and was vacuumed up in to a water truck.
1443026095 2015-09-23 Production Wells No Violations On the day of my inspection I found Buckeye Oil crew ready to remove tanks from the tank battery. There is not necessary to re- meditate any more of the tank battery. The bottom of the tanks are ok. The bottom of the one tank did develop a leak when moving the tank. Both of the 210 bbl tanks are being removed from location and replaced with one 100 bbl tank. On parts of the location lime is being used to help stabilize any left over oil stains or pockets of oil. I will be meeting with Buckeye Oil on Friday morning, to evaluate location on if other measures are necessary, If none are needed we will seed and mulch area that do not need to be disturbed again.
1443443094 2015-09-25 Production Wells No Violations Upon arrival on location I met with, Trisha Cartmell with ODNR, John Wellman Environmental Enforcement, Ken Mettler, Investigator with Ohio EPA. A discussion was held on the recent Oil spill and as to the cause.
1443458474 2015-09-28 Production Wells No Violations On the day of my inspection I found that Buckeye Oil has continued their cleanup. The part by the road ditch is ready for seed and mulch. Some of the absorbent pads can be removed and the booms need to stay in place for now. This site will still need to be monitored.
1443617371 2015-09-30 Production Wells No Violations On the day of my inspection this a follow up to the continues cleanup. The seeding and mulching has been completed. Some of the boom that are no longed effective need to be removed, and some of the absorbent pad need replaced and some are no longer need. I called Ronnie with Buckeye Oil and discussed what is needed to be done.
1444134377 2015-10-06 Production Wells No Violations This is a follow inspection. The clean up continues. Seeding and mulching is complete. Booms and absorbent pads are still installed and will need to be monitored. The tanks have been removed and replaced with one 100 tank.
1446121981 2015-10-29 Production Wells No Violations This is a follow up inspection to a previous oil spill. The are continues to improve. The grass is growing to re-establish the vegetation. The absorbent pads need to be replaced after the heavy rain. The boom is still in place at the culvert before it crosses the road into the wetlands. I called Ronnie with Buckeye Oil and let him know of the work thats needed to be done. I will continue to monitor.
1446646551 2015-11-04 Production Wells No Violations Inspector Akins and I are on site today accompanied by Natural Resources Engineer Technician Randy Householder, to assist in the reclamation of a strip mine(Chestnut Ridge East) that is close to this historical well. Householder and Akins came to an agreement on the concerns of the well and the planning of what should be done for the reclamation of the area around this well site.
1451485398 2015-12-30 Production Wells No Violations This is a follow up inspection to a earlier oil spill. Look at the area that was impacted I found no sign of any oil, the area has received a lot of rain over the last few days. The clean up was successful and no further maintance is needed.
