The well was capable of production and it was equipped with 4.5 inch casing; 1.5 inch tubing and a plunger lift system. The production equipment included a 100bbl tank, a concrete brine vault not in use, a horizontal separator, a drip and a compressor unit all within an earthen dike. |

The owner/operator shall properly identify well with correct information, remove debris, repair band clamp on 4.5 inch casing, clean-up contamination inside the dike and as instructed complete the clean-up at the well head. |
