0310597995 | 2017-11-09 | Production Wells | No Violations | On the date of my inspection, I found a well maintained lease road wellhead equipped with a pumping unit, rods, 2.375 inch tubing, and 4.5 inch casing. There is an identification tag on the pumping unit. The tubing and casing are tied into a 2.375 inch production line. The tubing and casing valves are now in the closed position. The production equipment shared with permit numbers 34-157-2-3641, 34-157-2-3642, 34-157-2-3643, 34-157-2-3639, 34-157-2-3771, and 34-157-2-4251 includes 4 horizontal separators (2 have chart boxes with charts), two 100 barrel tanks, and one 50 barrel poly brine tank within an earthen berm. There is proper identification on the tanks. Currently all of the gas produced by these wells is being vented through the tanks. The flowline for this well is leaking on the driveway near the production tanks. Oil and gas are bubbling out of the gravel. The contaminated area is approximately 8 feet by 6 feet in size. I spoke with the pumper as well as Everflow Eastern Corp (330-507-6975), and George Strawn (330-727-1614) about the issues. They shut in the wells and are sending a clean up crew out this afternoon to clean up the flowline leak. I informed them that the gas must either be captured and sold or flared, and cannot be freely vented to the atmosphere. |
1194719618 | 2018-03-19 | Production Wells | No Violations | Upon arrival on location I met up with Tom Danial, the property owner, Jason Blocker with ODNR Emergency Response team, and George with Everflow. The electric motor had to be replaced and the well was then turned on and pumped. The well was pumped up to 300 psi and we found no leaks. The Flow line was then pressured up to 100 psi, I then walked the flowline to the storage tanks and I found no leaks. George with Everflow was instructed on what cleanup needed to be done. I also gave George a verbal notice for pollution and contamination. The cleanup is to start tomorrow March 20, 2018. |

In surveying the area, we found spots of crude oil around the wetland area. There was oil film on the well pumping unit that was covered with water when the area was flooded. There are rings of oil on the trees that was left after the water levels went down. Other wells were check in the area, and there was no indication that anything came from them. The oil was isolated to a 30 acre piece of ground. It was decided that more testing was needed and was to be done on Monday March 19, 2018. |

The stuffing box seals have been replaced and the 8 5/8 inch casing valve is closed. The vegetation that was covered with oil has been cut and removed from the location. Some of the trees have been wiped down. There are still areas that need cleanup. More follow-up inspections are needed. |
