0277814063 2015-05-19 Production Wells No Violations On the date of inspection I found production equipment to include a 210bbl tank and a concrete brine vault within a partial earthen dike. A horizontal separator is located outside of the dike. The well is equipped with a pumping unit, rods, 2.375 inch tubing, 4.5 inch casing, and 8.625 inch casing. The tubing valve and one casing valve is in open position. Identification is present and production records are current through 2014. At the time of my inspection I found that there was between 5-10 gallons of product on the ground around the well head. I left a message with the Northwood Energy office and should be receiving a call back from a superintendent.
0570794361 2018-07-12 Production Wells No Violations Upon arrival on location, I witnessed the well was equipped with 4.5 inch diameter casing, 2.375 inch diameter tubing, rods, and a pumping unit operated with an electric motor. The well was running at the time of the inspection. Production equipment included a steel 210 barrel tank and a concrete vault were inside an earthen dike, and a horizonal separator. Domestic gas was being supplied off the separator. The earthen dike area was being shared with a tank from the Miracle-Crim 2 (2001). The production report was up to date and proper identification was in place.
0726456785 2019-03-12 Production Wells No Violations Upon my arrival on location, I witnessed the well location had been smoothed up, along with the lease road. The well head was free from any leaks. The well was put back into production. Talked to Chris Abule about complaint 5976. He stated that he has not smelled any gas for a month or longer. I told Chris I would be closing this complaint. I will contact Northwood, the well owner, about this complaint.
0875726395 2018-12-13 Production Wells No Violations This inspection was a follow up to the complaint 5976. Northwood has been and are still working on the wellhead. They straightened the lease road up with a dozer and they cleaned up the location, around the pumping unit. They are now focused on replacing fitting on the wellhead and putting new wellhead rubbers in the 4.5 inch diameter and 8.625 inch diameter wellheads. They will inform me when they are ready to put the well back into production.

1001287094 2017-09-18 Temporary Abandoned Annular Disposal No Violations On the day of my inspection I found the well equipped with 8 5/8 inch diameter casing, 4 1/2 inch diameter casing, and 2 3/8 inch diameter tubing and rods, operated by a pumping unit with an electric motor. There is no equipment hookup for annular disposal. The production equipment is made up of one 210 bbl. steel tank, a horizontal separator, and a recording gauge. The production reporting is up to date. The tank is properly identified. The well provides domestic gas.
1541526547 2018-11-06 Production Wells No Violations This inspection is a follow up to complaint 5976 on a smell of gas on 9/28/18 by neighbors. Upon my arrival on location, I witnessed the well owner has replaced some fittings on the wellhead and has tightened the wellhead rings. He put new rubbers in the stuffing box and replaced the polish rod. He is going to treat the 8.625 inch diameter casing with sulfur treat by the end of the week. He is going to clean the old oil up, from around the wellhead too. The well has been shut in since the complaint came in from neighbors. I told him about a leak I found along the lease road, this morning. He will have his well tender take look at the leak. If, it is not their line, he will call Enervest about the leak. He will keep me updated on all the progress he makes.
