0833300994 2018-01-04 Plug / Plug Back No Violations On the day of my inspection I found that the 8 5/8 inch diameter casing has been cut off and platted.
1372150274 2015-04-08 Production Wells No Violations On the date of inspection I was accompanied by ODNR Inspector Randy Akins to verify the location and status of the well. The well is equipped with a gas plunger lift system, 1.5 inch tubing, 4.5 inch casing, and 8.625 inch casing. There is production equipment approximately 2500 feet away with the permit number for this well located on it. The production equipment includes a 100bbl tank, 30bbl plastic tank, and a horizontal separator within a fence and earthen dike. All valves are in closed position at the well head, the flow line is disconnected, and various equipment has been removed. The well head is fenced in and identification is present. Production has been reported through 2014.

1527251584 2018-05-25 Final Restoration No Violations On the day of inspection, I found that the final restoration inspection has been completed. The site passed final restoration according to ORC 1509.072 (B).
1632845194 2017-12-13 Plug / Plug Back No Violations I was on location to observe the P&A activities with Randy Akins. Bishop Well Service (BWS) was the plugging contractor and Petroset Cementing Services (PCS) was the cement contractor. BWS removed the 1.5 tubing from the 4.5 production casing to drop the mud anchor and assess its suitability for use as a work string. BWS lowered the 1.5 tubing back into the 4.5 casing. PCS established circulation and pump a 30 sack slurry of Class A, blended with 2% Gel, for the Clinton sand plug. Well was shut in overnight.
