0000PL2358 | Preliminary Restoration | No Violations | Preliminary Restoration done information from Data Point | |
0711898773 | 2018-09-18 | Production Wells | No Violations | Upon arrival on location, I witnessed the well was equipped with 4.5 inch diameter casing, 1.5 inch diameter tubing, and a plunger lift system. The valves on the tubing and on the casing were open. Production equipment included a steel 100 barrel tank, a horizonal separator, and a chart recorder with a 2 by .750 inch orifice plate within an earthen dike. The production report was up to date and proper identification was present. The inside of the earthen dike had new gravel put down and a used, steel 100 barrel was plumbed into the separator. The outside of the containment was cleaned up and had vegetation starting to grow on it. |
1534543247 | 2018-08-17 | Production Wells | No Violations | Received a phone call from Andrew Adgate about a lightning strike on an oil tank in Tuscarawas County. Upon my arrival on location, accompanied by Jason Blocker, I witnessed a steel 210 barrel tank had been hit by lightning. The fire department had the fire contained. The owners of the well were on site too. The steel 210 barrel tank was outside the earthen dike, laying 75 yards up the hill. 92% of the oil in the tank burnt up inside the earthen dike. 6% of the oil in the tank was splashed outside the earthen dike on the initial explosion. That 6% of oil burnt up by the fire too. The 2% of oil from the tank was being cleanup by the employees of the well owner. They were using oil absorbent pads and oilpeat absorbent. The well owner was already cleaning the location with an excavator, dozer, and water truck. They were scraping the contaminated dirt into piles. These piles were being put into a rolloff box that had been delivered to location already. They had installed precautionary oil booms in a near by creek down over the hill. This was just in case of run off from the rain we were receiving. Clean up was 50% done by the time I wrote up my report.
I witnessed the well was equipped with 4.5 inch diameter casing, 1.5 inch diameter tubing, and a plunger lift system. The well was shut in at the time of the inspection, because of the lightning strike on the tank. Production equipment included a steel 210 barrel tank, a horizonal separator, and chart recorder within an earthen dike. The production report was up to date and proper identification was in place. |
1534768599 | 2018-08-20 | Production Wells | No Violations | Upon my arrival on location, I witnessed the well owner had the area outside the earthen dike cleaned up. The contaminated dirt was hauled away and was reclaimed with fresh dirt. The earthen dike area will need reclaimed and a new tank has not been installed yet. The old steel 210 barrel was not removed from the location. |
1847914482 | 2016-08-18 | Production Wells | No Violations | On the day of my inspection I found an 8 5/8 X 4 1/2 inch wellhead, and a 4 1/2 X 2 3/8 inch wellhead operated by a plunger lift system. The valves are in the open position. The tank battery is made up of one 210 bbl. tank, a horizontal separator, and a recording gauge, all in an earthen dike. The production reporting is up to date, the tank is properly identified, and there are no violations at this time. |
